101.04 Definitions.
Delete the following definitions and substitute the following:
Adjustment in Contract Price--“Equitable adjustment,” as used in the Federal Acquisition Regulations, or “construction cost adjustment,” as used in the Timber Sale Contract, as applicable.
Bid Schedule--The Schedule of Items.
Bridge--No definition.
Change--“Change” means “change order” as used in the Federal Acquisition Regulations, or “design change” as used in the Timber Sale Contract.
Clearing Limits—The limits on each side of the project that establish the area disturbed by construction operations beyond which no disturbance is permitted. Typically the clearing limits http://www.nist.gov/ are two feet beyond the cut and fill limits shown on the Plans, except when additional clearing of hazard trees or other hazardous obstacles are required to be cleared.
Contractor--The individual or legal entity contracting with the Government for performance of prescribed work. In a timber sale contract, the contractor is the “purchaser”.
Culvert--No definition.
Design Quantity--“Design quantity” is a Forest Service method of measurement from the FS-96 Forest Service Specifications for the Construction of Roads and Bridges. Under these FP specifications this term is replaced by the term “Contract Quantities”.
Forest Service--The United States of America, acting through the Forest Service, U.S.
Department of Agriculture.
Neat Line--A line defining the proposed or specified limits of an excavation or structure.
Pioneer Road--Temporary construction access built along the route of the project.
Purchaser--The individual, partnership, joint venture, or corporation contracting with the Government under the terms of a Timber Sale Contract and acting independently or through agents, employees, or subcontractors.
Protected Streamcourse--A drainage shown on the plans or timber sale area map that requires designated mitigation measures.
Right-of-Way--A general term denoting (1) the privilege to pass over land in some particular line (including easement, lease, permit, or license to occupy, use, or traverse public or private lands), or (2) Real property necessary for the project, including roadway, buffer areas, access, and drainage areas.
Road Order--An order affecting and controlling traffic on roads under Forest Service jurisdiction. Road Orders are issued by a designated Forest Officer under the authorities of 36 CFR, part 260.
Schedule of Items--A schedule in the contract that contains a listing and description of construction items, quantities, units of measure, unit price, and amount.
Utilization Standards--The minimum size and percent soundness of trees described in the specifications to determine merchantable timber.

(c) As-Built-Plans. Furnish one set of as built plans. The Government will provide one set of contract plans to be used exclusively for recording the as-built details of the project. Use red pencil or red ink to record the information on the as-built plans.
Note all additions or revisions to the location, character, and dimensions of the prescribed work shown on the contract plans. Line out all details shown that are not applicable to the completed work. Check off details shown that were incorporated into the completed work without change.
Retain the plans at the project site and, as work progresses, continually update them to reflect the as-built details. Upon request, make the plans available to the CO to review for compliance with these specifications.

104.06 Use of Roads by Contractor
The Contractor is authorized to use roads under the jurisdiction of the Forest Service for all activities necessary to complete this contract, subject to the limitations and authorizations designated in the Road Order(s) or described in the contract, when such use will not damage the roads or national forest resources, and when traffic can be accommodated safely.
105 - Control of Material

105.01 Source of Supply and Quality Requirements.
All materials incorporated into the project shall be certified noxious weed seed free, either through visual inspection or commercial certification.
105.02 Material Sources.
(a) Government-provided sources.
Comply with the requirements of 30 CFR 56, subparts B and H.
The following sources have been designated for this project:
Waste Site – Government Furnished – The Waste Disposal Site for excess, unsuitable, and boulder material is located within an existing pit along the north and west sides of the designated Staging Area for the project construction (Section 35, Township 10N, Range 13E).
Excess, unsuitable, and boulder materials, herein called waste material, hauled to this site shall be dumped inside the pit where designated by the Contracting Officer. Shape and compact the waste material in its final location and restore to match original contours prior to its original pit development. Compaction effort shall be such to ensure minimal-to-no settlement of waste material following completion of the work. Ensure boulder material, if applicable, is positioned toward the bottom of the fill hole, with finer graded material being on the surface. Where larger boulders exist, work in small rock and fines to fill voids. Shape material to provide positive drainage in all locations.
Select Borrow – Government Furnished – The material for Select Borrow is located in Section 21, Township 8N, Range 14E, at the Champion Creek Pit located approximately 12 miles south of this project via State Highway 75.
(b) Contractor-located sources.
Optional Non-Government Source for this project is as follows:
Materials for use as Crushed Aggregate Base Course, Gravel Borrow, and Pipe Zone Bedding/Backfill may be obtained from the Fourth of July Creek Pit which is managed by the Idaho Transportation Department (ITD), and is located approximately 12 miles south of this project via State Highway 75.
The Contractor is responsible for all applicable production royalty fees required of materials and all other costs related to the work that may be required for materials obtained from this source including but not limited to overburden stripping and rock crushing/processing. Contact Lynn White at ITD (208.886.7800) for additional information.
Prior to any pit development or use of materials, the pit site shall be approved by the CO as free of noxious weed as listed on the "All States Noxious Weeds List”.
105.05_nat_us_05_12_2004
105.05 Use of Material Found in the Work.
Delete 105.05 (a) and (b) and the last sentence of the second paragraph and substitute the following:
Materials produced or processed from Government lands in excess of the quantities required for performance of this contract are the property of the Government. The Government is not obligated to make reimbursement for the cost of producing these materials.
106 - Acceptance of Work
106.01_nat_us_07_31_2007
106.01 Conformity with Contract Requirements.
Delete Subsection 106.01 and substitute the following:
Follow the requirements of FAR Clause 52.246-12 Inspection of Construction.
References to standard test methods of AASHTO, ASTM, GSA, and other recognized standard authorities refer to the methods in effect on the date of solicitation for bids.
Perform all work to the lines, grades, cross-sections, dimensions, and processes or material requirements shown on the plans or specified in the contract.
Incorporate manufactured materials into the work according to the manufacturer’s recommendations or to these specifications, whichever is more strict.
Plan dimensions and contract specification values are the values to be strived for and complied with as the design values from which any deviations are allowed. Perform work and provide material that is uniform in character and reasonably close to the prescribed value or within the specified tolerance range. The purpose of a tolerance range is to accommodate occasional minor variations from the median zone that are unavoidable for practical reasons.
When standard manufactured items are specified (such as fence, wire, plates, rolled shapes, pipe conduits, etc., that are identified by gauge, unit mass, section dimensions, etc.), the identification will be considered to be nominal masses or dimensions. Unless specific contract tolerances are noted, established manufacturing tolerances will be accepted.
The Government may inspect, sample, or test all work at any time before final acceptance of the project. When the Government tests work, copies of test reports are furnished to the Contractor upon request. Government tests may or may not be performed at the work site. If Contractor testing and inspection is verified by the Government, the Contractor’s results may be used by the Government to evaluate work for acceptance. Do not rely on the availability of Government test results for process control.
Acceptable work conforming to the contract will be paid for at the contract unit bid price. Four methods of determining conformity and accepting work are described in Subsections 106.02 to
106.05 inclusive. The primary method of acceptance is specified in each Section of work.
However, work may be rejected at any time it is found by any of the methods not to comply with the contract.
Remove and replace work that does not conform to the contract, or to prevailing industry standards where no specific contract requirements are noted, at no cost to the Government.
(a) Disputing Government test results. If the accuracy of Government test results is disputed, promptly inform the CO. If the dispute is unresolved after reasonable steps are taken to resolve the dispute, further evaluation may be obtained by written request.
Include a narrative describing the dispute and a proposed resolution protocol that addresses the following:
(1) Sampling method;
(2) Number of samples;
(3) Sample transport;
(4) Test procedures;
(5) Testing laboratories;
(6) Reporting;
(7) Estimated time and costs; and
(8) Validation process.
If the evaluation requires additional sampling or testing be performed, mutually agree with the Government on witnessing procedures and on sampling and testing by a third party laboratory. Use a third party laboratory accredited by the AASHTO accreditation program.
Provide proof of the laboratory’s accreditation for the test procedures to be used. Do not use the same laboratory that produced the disputed Government test results or that produced the test results used as a basis for the dispute.
The CO will review the proposed resolution protocol and may modify it before final approval and execution.
The Government will use the approved resolution protocol test results to determine the validity of the disputed testing. If the Government test results are validated, the Contractor will be responsible for all costs associated with developing and performing the resolution protocol. If the Government test results are not validated, the Government will be responsible for all costs associated with developing and performing the resolution protocol.
If the validity of the Government test results cannot be determined, the Contractor and Government will equally share all costs associated with developing and carrying out the resolution protocol.
(b) Alternatives to removing and replacing non-conforming work. As an alternative to removal and replacement, the Contractor may submit a written request to:
(1) Have the work accepted at a reduced price; or
(2) Be given permission to perform corrective measures to bring the work into conformity.
The request must contain supporting rationale and documentation. Include references or data justifying the proposal based on an evaluation of test results, effect on service life, value of material or work, quality, aesthetics, and other tangible engineering basis. The CO will determine disposition of the nonconforming work.

107 - Legal Relations and Responsibility To the Public

107.01 Laws to be Observed.
Delete the third paragraph and replace with the following:
Comply with the terms and conditions included in all permits and agreements for performing the work included in this contract. Notify the CO immediately of any changes or any additional permits or agreements that are required by the Contractor’s methods of operation. Allow adequate time in the construction schedule for any additional permits and agreements not in the contract.
The Contractor shall prepare a Storm Water Pollution Prevention Plan (SWPPP) for this project in accordance with current EPA requirements. The Contractor shall comply with all terms and conditions in the SWPPP and shall be required to update the SWPPP to ensure that it remains in compliance with applicable permits and other legal requirements. The Contractor shall be responsible for carrying out all duties outlined in the SWPPP and as required with any updates.
Government’s role will be that of oversight to ensure Contractor compliance with SWPPP.
Comply with the requirements of the Fire Protection and Suppression Plan, and other permits/plans included in this contract.
107.02_sawtooth_01_15_2017
107.02 Protection and Restoration of Property and Landscape.
Tree removal and felling work associated with constructing the new paved parking lot and trail and required vegetation removal for the Champion Creek Pit Material Source expansion, if required, shall only occur following approval from both the Sawtooth National Forest Wildlife Biologist and from the Contracting Officer. The Contractor shall coordinate site inspection with Forest Wildlife Biologist prior to any tree removal, felling, or Champion Creek vegetation removal work. If active nests are encountered during this pre-clearing site inspection, the Contractor shall halt all operations within the perimeter established by the Forest Wildlife Biologist. Work will resume in the established perimeter either when the nests have been relocated by the Forest, the birds have fledged, or when other mitigations have been established by the Forest Wildlife Biologist.

Perform all operations in a prudent, conscientious, safe and professional manner. Ensure that all personnel involved in handling and packaging the hazardous waste are trained for the level of expertise required for the proper performance of the task and, in particular, in the areas of chemical incompatibility, general first aid procedures, and spills. Provide handling and personal protective equipment appropriate to ensure safe handling of the hazardous waste according to 29 CFR 1910.120). Notify the Forest Service of all hazardous material that may be brought onto the National Forest.

107.10 Environmental Protection.
Design and locate equipment repair shops, stationary refueling sites, or other facilities to minimize the potential and impacts of hazardous material spills on Government land.
Before beginning any work, submit a Hazardous Spill Plan. List actions to be taken in the event of a spill. Incorporate preventive measures to be taken, such as the location of mobile refueling facilities, storage and handling of hazardous materials, and similar information. Immediately notify the CO of all hazardous material spills. Provide a written narrative report form no later than 24 hours after the initial report and include the following:
• Description of the item spilled (including identity, quantity, manifest number, and other identifying information).
• Whether amount spilled is EPA or state reportable, and if so whether it was reported, and to whom.
• Exact time and location of spill including a description of the area involved.
• Containment procedures.
• Summary of any communications the Contractor had with news media, Federal, state and local regulatory agencies and officials, or Forest Service officials.
• Description of clean-up procedures employed or to be employed at the site including final disposition and disposal location of spill residue.
When available provide copies of all spill-related clean up and closure documentation and correspondence from regulatory agencies.
The Contractor is solely responsible for all spills or leaks that occur during the performance of this contract. Contractor shall clean up spills or leaks to the satisfaction of the CO and in a manner that complies with Federal, state, and local laws and regulations.
Contractor shall provide all necessary measures to ensure no concrete or paint enters any live stream.
107.11_nat_us_02_17_2005
107.11 Protection of Forests, Parks, and Public Lands:
Before starting any operations on the project, the Contractor shall prepare & submit a Project Fire Plan, in cooperation with the Contracting Officer, providing for the prevention and control of fires in the project area. The Contractor shall certify compliance with all fire protection and suppression requirements, before beginning operations during the fire period and closed season.
Required Tools and Equipment shall be kept currently in serviceable condition and immediately available for initial attack on fires.
108 - Prosecution and Progress

108.01 Commencement, Prosecution, and Completion of Work.
Replace the Subsection with the following:
Perform work under this contract according to the following:
(a) For construction operation occurring up to the Thursday prior to Memorial Day, the Contractor can work any time during the week;
(b) For construction operations occurring between the Tuesday following Memorial Day and Labor Day the Contractor shall only be able to work from 8:00 a.m. to 8:00 p.m., Mondays through Thursdays and 8:00 a.m. to 5:00 p.m., on Fridays;
(c) No construction operations will be permitted between 5:00 p.m. Friday and 8:00 a.m. the following Wednesday for Independence Day;
(d) No construction operations will be permitted between 5:00 p.m. Friday, August 18, 2017 and 8:00 a.m. Monday, August 28, 2017 for the Great American Eclipse that will occur on August 21, 2017; and;
(e) For construction operations occurring after the Tuesday following Labor Day, the Contractor can work any time during the week.
General work sequence or procedures:
(a) Complete all earth moving activities to render the New Road and new Outlet CG Access built up to subgrade, including construction of roadside ditches and all related drainage improvements by the Fall, 2017;
(b) Complete remaining road construction work for the New Road and new Outlet CG Access prior to permanently switching traffic onto these new roadways. Removal of existing asphalt will be permitted prior to this permanent traffic switch;
(c) Following this permanent traffic switch, complete the remaining parking lot/trail removal and reconstruction, and decommission the old roadways and parking lot. Access to the Outlet CG shall be maintained at all times.
(d) Complete all remaining work by September 21, 2018.

With the exceptions of Section 151 – Mobilization and Section 635 – Temporary Traffic Control, lump sum progress payments shall reflect the percentage of lump sum work completed, as based on lump sum breakdowns submitted by the Contractor to the CO for review and approval prior to the Pre-Work meeting.

151 - Mobilization

Description
151.01 This work consists of moving personnel, equipment, material, and incidentals to the project and performing all work necessary before beginning work at the project site; obtaining of permits, insurance, and bonds; providing sanitary facilities for the Contractor’s personnel; and performing all other required work necessary before beginning work on the Contract.
This work also includes washing and treating construction equipment and vehicles necessary for equipment transport to remove seeds, plants, and plant fragments before the equipment is used on Forest Service lands, according to the requirements within.
Construction Requirements
Wash the sides, tops, and undercarriages of all construction equipment. Remove all seeds, plants, plant fragments, dirt, and debris from the construction equipment. Only equipment inspected by the Forest Service will be allowed to operate within the project area. All subsequent move-ins of equipment to the project area will be treated in the same manner as the initial move-in. This requirement does not apply to cars, pickup trucks, and other vehicles that regularly travel between the construction site and areas off the National Forest.
Equipment will be considered free of soil, seed, and other such debris when a visual inspection does not disclose such material. Disassembly of equipment, components or the need for specialized inspection tools is not required.
Notify the CO in writing at least 72 hours before moving any construction equipment onto the National Forest. Notification will include an agreed upon location where the equipment will be available for inspection by the Forest Service. Inspection will be required after every cleaning.
Use methods of cleaning and locations for cleaning approved by the CO.
For work at a commercial washing facility, use an approved facility.
New infestations of noxious weeds of concern to Forest Service and identified by either Contractor or Forest Service, in the Project Area or on the haul route, will be promptly reported to the other party. Contractor and Forest Service will agree on treatment methods to reduce or stop the spread of noxious weeds when new infestations are found. A current list of noxious weeds of concern to Forest Service is available at each Forest Service office.
Measurement
151.02 Clean equipment prior to moving onto this project. The initial cleaning will not be included in the measurement for payment. Subsequent cleanings necessitated by the Contractor’s actions will not be included in the measurement for payment.
Measure mobilization according to Subsection 109.02.
Payment
151.03 The accepted quantity, measured as provided in Subsection 109.02, will be paid at the contract price per unit of measurement for the Section 151 pay item shown in the bid schedule.
Payment will be full compensation for the work prescribed in this Section. See Subsection 109.05.
Progress payments for mobilization lump sum will be paid as follows:
(a) If applicable, bond premiums will be reimbursed according to FAR Clause 52.232-5 Payments Under Fixed-Price Construction Contracts, after receipt of the evidence of payment.
(b) When 5 percent of the original contract amount is earned from other bid items, 50 percent of the mobilization item, or 5 percent of the original contract amount, whichever is less, will be paid.
(c) When 10 percent of the original contract amount is earned from other bid items, 100 percent of the mobilization item, or 10 percent of the original contract amount, whichever is less, will be paid.
(d) Any portion of the mobilization item in excess of 10 percent of the original contract amount will be paid after final acceptance.
Include all costs associated with the cleaning of equipment in the unit bid price for Mobilization.
152 - Construction Survey and Staking

Delete Section 152 in its entirety and replace with the following.
152.01 This work consists of performing surveying, staking, calculating, and recording data for the control of work. See FAR Clause 52.236-17 Layout of Work.
Construction Requirements
152.02 Qualifications. Provide technically qualified survey crews experienced in highway construction survey and staking. Provide personnel capable of performing in a timely and accurate manner.
152.03 Submittals. Submit the following at the preconstruction conference:
Include the following when Automated Machine Guidance (AMG) methods are used:
(a) Technology statement. A written statement that includes:
(1) The manufacturer, model, and software version of the AMG equipment; and
(2) Certification that the final 3D data is compatible with the AMG equipment.
(b) Personnel qualifications.
(1) The name, authority, relevant experience, and qualifications of the person with overall responsibility for the AMG system.
(2) The names, authority, and relevant experience of personnel directly responsible for operating the AMG equipment.
(c) Contractor quality control plan. Comply with Section 153 and describe procedures for checking, mechanical calibration, and maintenance of both survey and construction equipment. Include the frequency and types of checks performed.
Include a price breakdown by individual tasks when construction survey and staking is paid by the lump sum for use in making progress payments and price adjustments.
152.04 General. Conform to the following:
(a) Personnel. Provide a crew supervisor on the project whenever surveying and staking is in progress.
(b) Equipment. Furnish survey instruments and supporting equipment capable of achieving the specified tolerances.
Construction equipment controlled with a Global Positioning System (GPS) and Robotic Total Station (RTS) machine guidance system may be used in the construction of subgrade, subbase, and base aggregate courses, or other construction operations when approved.
(c) Material. Furnish acceptable tools and supplies of the type and quality suitable for highway survey work. Furnish stakes and hubs of sufficient length to provide a solid set in the ground with sufficient surface area above ground for necessary legible and durable markings.
Use required stake dimensions and materials. Pre-paint the top 2 inches of all stakes and lath, or mark them with plastic flagging. Use approved colors, prior to staking, for paint or flagging. Mark all stakes with a stake pencil that leaves a legible imprint, or with waterproof ink.
Do not use aerosol spray paints.
Include staking activities in the construction schedule required under Section 155. Include the dates and sequence of each staking activity.
The Government will set horizontal control points, vertical control points, and will provide data for use in establishing control for completion of each element of the work.
Data relating to horizontal and vertical alignment, theoretical slope stake catch points, and other design data will be furnished. Reformatting and additional calculations may be required for the convenient use of the Government-furnished data. Provide immediate notification of apparent errors in the initial staking or in the Government-furnished data.
Record survey and measurement field data in an approved format. Submit as-staked data and corrections made to the Government-furnished survey data. Submit survey and measurement data at least weekly. Field data and supporting documentation become the property of the Government upon completion of the work.
Discuss and coordinate the following with the CO before surveying or staking:
(1) Surveying and staking methods;
(2) Stake marking;
(3) Grade control for courses of material;
(4) Referencing;
(5) Structure control;
(6) Field staking data;
(7) Localization of the GPS systems to the Government-established control points; and
(8) Other procedures and controls necessary for the work.
Do not start work until staking or three-dimensional (3D) verification data for the affected work has been approved.
The Forest Service will provide a plan sheet that shows the locations, elevations, and descriptions of control points set during the project design phase along with any points in electronic format. It is the Contractor’s responsibility to verify the location and elevation information for each of these points and report any discrepancies or missing points to the CO immediately. Following verification of the point data, the Contractor shall stake out the project improvements. Further plan sheets containing data relating to horizontal and vertical alignment, theoretical slope stake catch points, and other pertinent design data will be provided. All survey stakes shall be approved by the CO prior to building proposed improvements. The Contractor shall notify the CO ten (10) days prior to the intended beginning of construction for approval of stakes. It is the Contractor's responsibility to perform any further staking, surveying, and control as required once construction has commenced to ensure all the required project elements are built in accordance with the Contract Documents.
The Contractor shall reestablish missing reference, control lines, or stakes as necessary to control subsequent construction staking operations during the course of the project construction.
Acceptance of the construction staking does not relieve the Contractor of responsibility for correcting errors discovered during the work and for bearing additional costs associated with the error.
Maintain legibility of stake markings for the duration of the project or until notified in writing the stakes are no longer needed. Replace stakes if necessary to ensure markings are maintained.
Remove and dispose of flagging, paint, lath, stakes, and other staking material after the project is complete.
152.05 Survey and Staking Requirements. Perform survey, staking, recording of data, and calculations as necessary to construct the project from the initial layout to final completion.
Survey and set stakes to the precisions and tolerances in Tables 152-1 and 152-2, respectively.
Reset stakes, refine 3D data, or both as many times as necessary to construct the work.
(a) Control points. Relocate initial horizontal and vertical control points in conflict with construction to areas that will not be disturbed by construction operations. Furnish the coordinates, elevations, and supporting documentation for the relocated points before the initial points are disturbed. Set durable monuments for survey control that uniquely identify the points.
Furnish the GPS localization results at least 7 days before beginning construction layout survey work. The CO may order the GPS localization calibration and associated 3D model to be broken into two or more zones to maintain the localized relationship between control points and original ground.
(b) Centerline establishment. Establish or reestablish centerline at roadway design cross-section locations and as necessary to construct the work. Reestablish the centerline when construction survey and staking work does not meet the tolerances.
(c) Original ground topographic verification. Use an approved method to regenerate cross-section data in areas where theoretical and actual ground elevations do not meet a tolerance of plus or minus 0.5 feet. Retake cross-section to verify existing ground topography to mapping. Submit cross-section or 3D data in electronic and printed format for approval.
Reduce cross-sections to horizontal and vertical distances from centerline.
Retake cross-section 10 feet beyond catch points to verify existing ground topography.
(d) Slope and references stakes. Perform the following:
Use the daylight limit (cut/fill) line shown in the plans to calculate the actual location of the catchpoint. Daylight limit lines are based on template information shown in the plans, however, some variation exists where the daylight line was manually adjusted during the design to better blend with existing terrain. Slope stakes shall be marked with station, the amount of cut or fill, the horizontal distance to centerline, and the slope ratios.
(1) AMG method. After clearing operations are completed, set centerline reference stakes and hubs on both sides of centerline at 100-foot intervals at the clearing limit locations. When curves exist in the alignment, reduce centerline reference stake and hub interval to CO’s satisfaction. Where clearing limits are greater than 10 feet vertically, 25 feet horizontally, or both from subgrade hinge point; provide an additional reference stake and hub as approved by the CO. Label each centerline reference stake with station, hub elevation, and offset from centerline.
Provide as-built cross-sections at random locations specified by the CO not to exceed 500-foot intervals. If as-built cross-sections do not meet the tolerances in Subsection 204.13(d); rework the section until the specified tolerances are achieved and provide additional cross-sections as directed by the CO at no cost to the Government.
(2) Conventional survey methods. Verify and set slope stakes on both sides of centerline at the theoretical catch point. If the theoretical catch point is not within a tolerance of 0.5 feet, perform original ground topographic verification according to Subsection 152.05(c). Set the slope stake at the actual intersection of the design roadway slope with the natural ground-line. Set reference stakes outside the clearing limits.
Include reference points and slope-stake information on the reference stakes.
Establish slope stakes in the field as the actual point of intersection of the design roadway slope with the natural ground-line when theoretical catch point information is not available.
(e) Clearing and grubbing limits. Set clearing and grubbing limits on both sides of centerline based on the actual slope-stake locations.
(f) Grade-finishing stakes.
(1) AMG method. Verify the grade elevation and horizontal alignment of roadway grade-finishing operations. Use conventional survey methods at random locations specified by the CO, not to exceed 500-foot intervals. Submit 3D coordinates of grade-finishing quality control checks.
(2) Conventional survey methods. Set grade-finishing stakes for grade elevations and horizontal alignment, on centerline and on each shoulder at design roadway cross-section locations. Set stakes at the top of subgrade and the top of each aggregate course. Reset grade finishing stakes as many times as necessary to construct the subgrade and each aggregate course.
During turnout or pullout construction, set stakes on the centerline, on each normal shoulder, and on the shoulder of the turnout. In parking areas, set stakes at the center and along the edges of the parking area. Set stakes in ditches to be paved.
When the centerline curve radius is less than or equal to 250 feet, use a maximum longitudinal spacing between stakes of 25 feet. When the centerline curve radius is greater than 250 feet, use a maximum longitudinal spacing between stakes of 50 feet. Use a maximum transverse spacing between stakes of 20 feet. Use brushes or guard stakes at each stake.
(g) Culverts. Set culvert reference stakes at all culvert locations. Set a culvert reference stake on the centerline of the culvert 10 feet from each end or beyond the clearing limit, whichever is greater. Record the following on culvert reference stakes:
(1) Diameter, actual field measured length, and type of culvert;
(2) The vertical and horizontal distance from the reference stake to the invert at the ends of the culvert;
(3) Station of actual point where culvert intersects centerline.
When required, stake headwall for culverts by setting a hub with a guard stake on each side of the culvert on line with the face of the headwall. Perform this work after clearing is completed.
(h) Bridges. Set adequate horizontal and vertical control and reference points for bridge substructure and superstructure components. Establish and reference the bridge chord, bridge tangent, or control lines as specified on the bridge plans. Also establish and reference the centerline of each pier, bent, and abutment.
(i) Retaining walls and reinforced soil slopes. Survey and record profile measurements along the face of the proposed wall or reinforced soil slope at 5 feet and 10 feet in front of the wall or slope face. Take cross-sections every 25 feet along the length of the wall or reinforced soil slope and at major breaks in terrain within the limits designated by the CO.
Measure and record points every 25 feet and at major breaks in terrain for each cross-section.
Set additional references and control points to perform the work.
(j) Borrow and waste sites. Perform field work necessary for initial layout and measurement of the borrow or waste site. Establish site limits and clearing limits. Measure both original and final ground conditions and submit cross-sections as directed by the CO.
(k) Permanent monuments and markers. Perform survey and staking work necessary to establish permanent monuments and markers as described in Section 621 or reestablish monuments as described in Subsection 107.02.
(l) Miscellaneous survey and staking. Survey and stake other work (such as but not limited to guardrail, curb and gutter, turf establishment, utilities, excavation limits for structures, signs, delineators, and object markers, pavement markings, and steel gate posts) to the proper location and required tolerances. Propose staking increments for approval by the CO when not specified.

Do not measure re-establishing missing control points or stakes after construction operations have begun.
