About this file
This is a wage information attachment form for contract opportunity AG-2020-23 issued by the Wisconsin Department of Agriculture, Trade and Consumer Protection (DATCP) for the procurement of printed "Bulk Milk Tank Inspector" decals. The decals are specified as 3.75" x 2.5" red vinyl stickers with 4 mil thickness, permanent adhesive with split backer, black ink, and a clear coat laminate finish. DATCP requests bids for the purchase of these printed decals and intends to use the results to award a contract, though no guarantee exists regarding minimum purchase quantities or dollar amounts. Bids requiring minimum commodity orders will be rejected. The contract term includes renewal options with wage rate adjustments permitted at renewal dates.
Attachment E requires all bidders to submit a wage data sheet with their bid that documents average hourly wage rates for all production employees assigned to the work, including shop foremen but excluding management, custodial, clerical, warehouse, and delivery personnel. The form allows for contract price adjustments at renewal dates based on wage rate changes, with one half of one percentage point of the annual wage change applying to the contract bid rate; for example, a 6% wage increase would result in a 3% rate adjustment. The original number of employees in each wage classification must remain constant throughout the contract life for adjustment calculation purposes. Bidders must sign and date the completed wage data sheet and return it with their bid submission; failure to submit this documentation may result in ineligibility for rate adjustments. Bidders may designate their wage data sheet as confidential under Wisconsin Public Records Law.

ATTACHMENT E WAGE INFORMATION
BIDDER NAME: ___________________________________________________________
Bidder’s Signature Date
This form is used to calculate the contract price adjustments which are allowed at a contract renewal date. One half of one percentage of change in the average wage rate may be applied to the contract bid rate. For example, if the percentage of annual change is 6%, the wage rate will be 3%. Allowable adjustments are effective at the renewal date of the contract and apply to all orders written after that date. The bidder/contractor must submit a current wage data sheet with any request for adjustment and must supply more wage information if requested.
Adjustment Basis – All bidders must submit with their bid a data sheet of average wage rates for all production employees who are, or will be, assigned to work on the procurement being bid including shop foreman and employees with split responsibilities. Any personnel not assigned to the contract, i.e.: management, custodial, clerical, warehouse, part-time hand bindery, delivery, and building trades personnel, are not included. Fringe benefits and taxes are not included.
For adjustment purposes, the original number of employees in each classification shall remain the same during the life of the contract period even if the actual number of employees changes.
Failure to submit a wage data sheet may result in the bidder/contractor not being eligible for rate adjustments. Bidder/Contractor my designate their wage data sheet as confidential under Wisconsin Public Records Law.
Instructions:
1. Indicate the number of employees in each wage bracket. Include only shop employees – do not include management, clerical or custodial employees. Shop foremen are considered as shop employees, not management employees. Area of work and number of employees in each area of work shall remain constant for the life of the contract.
2. Sign and date this form and return with your bid submittal.
