About this file
The document is an Affirmation and Disclosure Form related to a state contract, focusing on service location and data management requirements. The form requires contractors to disclose the principal business location, service performance locations, and data storage locations for both the primary contractor and any potential subcontractors. Contractors must affirm their understanding of executive orders prohibiting offshore services, offshore data storage, and purchasing from Russian institutions or companies.
The form mandates that contractors have a continuous duty to disclose any changes in service or data location before, during, and after contract execution. The state retains the right to immediately terminate the contract if services are performed outside the United States, unless a specific waiver is obtained. The document includes signature lines for the contractor to acknowledge their understanding and authorization, with space provided for the contractor's name, title, and date of execution. The form is marked as revised in March 2022 and is a two-page document designed to ensure transparency and compliance with state regulations regarding service and data management.
