About this file
This is an Addendum to FAR 52.212-2 that tailors evaluation criteria for a commercial services Request for Quotes (RFQ) conducted under FAR Part 12. The solicitation concerns commercial services to be provided at a physical facility venue with specific operational and logistical requirements.
Price evaluation will assess whether proposed prices for all line items are fair and reasonable, with total evaluated price calculated as the sum of all priced line items. Technical evaluation assigns ratings of either "Acceptable" or "Unacceptable" based on three criteria: (1) Operational Compliance & Attestation, requiring a written statement confirming full compliance with all Performance Work Statement (PWS) terms, timelines, and requirements without exception; (2) Facility Suitability, requiring identification of a specific commercial venue located within a 40-mile road radius of Concord, NH, with documented capability to accommodate 120 adults and 10 children in general session, two 20-person breakout rooms, a counseling room, two childcare rooms, audiovisual equipment, Wi-Fi connectivity, and 100 parking stalls per PWS Section 5.1; and (3) Licensing & Post-Award Deliverable Commitment, requiring a signed attestation that the contractor and any subcontractors meet special qualifications in PWS Section 1.4.7 and will provide mandatory documentation including state licenses, background checks, CPR credentials, and insurance per Technical Exhibit 2 (Deliverables Schedule). Quotes are rated Unacceptable if they take exception to any PWS requirement, provide insufficient detail for compliance determination, propose a facility outside the geographic radius or lacking required space and infrastructure, or fail to provide the required written attestation regarding mandatory deliverables.
