About this file

This is an addendum to Invitation to Bid (ITB) No. S-10700-00015928 issued by the State of Oregon for Equipment Maintenance, Repair and Related Services related to bulk fuel storage and dispensing equipment. Addendum No. 1, approved for publication on June 18, 2026, revises Attachment D—Bid Form to incorporate a new column allowing bidders to specify the percentage markup above material costs for instances where new equipment and parts are required to complete service work at designated sites. The addendum replaces the original Attachment D—Bid Form in its entirety with Attachment D—Bid Form REV.1 to reflect this pricing structure modification.

The revision addresses a specific gap in the original bid submission requirements by establishing a clear mechanism for bidders to propose their markup pricing on materials and equipment necessary for maintenance and repair services. This change ensures competitive transparency regarding the cost of parts and equipment procurement, enabling the State of Oregon to evaluate total service costs comprehensively across all bidders prior to contract award.
