About this file
This document is Addendum Number 3 for a Request for Quotation (RFQ) No. DEP25*51 for the Carbondale (Hill) Portals Phase II project, issued by the West Virginia Department of Environmental Protection (WV DEP). The addendum includes updated project details, insurance requirements, and specific project parameters such as tree clearing to be performed between November 15th, 2025, and March 31st, 2026, and compliance with Build America, Buy America (BABA) federal requirements. The document provides clarifications on stone access specifications, channel lengths, and mine seal pipe installation, with an attached pre-bid sign-in sheet showing attendance from various construction and engineering firms.
The project has an Engineer's estimate of $626,650.00 and requires specific insurance coverage, including General Liability Insurance of $1,000,000 per occurrence and $2,000,000 aggregate, Automotive Liability Insurance of $2,000,000, and Professional Liability Insurance of $1,000,000. The contractor must name The Kanawha Gauley Coal and Coke Company and The Boone East Development Co., LLC as additional insureds. The project involves specific environmental considerations, such as preventing sediment-laden water discharge and cutting trees greater than 3" in diameter at breast height during a designated clearing season, indicating a focus on environmental management and site preparation for what appears to be a mining or infrastructure development project.
