Bridgeport (Tomes) Landslide
Project Description:
Pre-Law (1977) mineworks resulting in overburden consisting of cast material. No significant coal refuse was discovered in Geotechnical borings. Due to cast material methods of construction 1 slide area has been identified to be repaired.
1. The landslide is located roughly 1,700 feet from the landowner’s residence. The landowner uses this area for cattle grazing. The slide is roughly 1.6 acres in size, drops off initially about 25 feet at the scarp line, and has a 217 feet scarp line in the shape of a bowl.
a. The slide will be excavated to bedrock.
b. Toe-Key, Bonding Benches, and Underdrains will be installed as directed on the plans & specifications
c. Excavated material will be placed in compacted lifts to the limits and configuration shown on the plans.
d. Approximately 1,940 linear feet of riprap channels will be installed in the configuration shown on the plans.
e. Additional culvert improvements and access road improvements are also included.
f. Contractor will be required to coordinate temporary fencing with the landowner.
2. Special Considerations: Please pay particular attention to the following Special Considerations. Failure to follow these procedures may result in “Stop Work” directives.
a. Residential Community
i. Special care and consideration to residents and their access, property, and utilities should be observed.
ii. The installation methods of residential utilities are unknown. Special handling in the form of steel plate protection is proposed. Any unplanned interruption of utility service to residents will be treated with highest priority to restore.
b. Private Driveway will be maintained throughout construction, suitable for access by landowners. Temporary fencing and cattle gate(s) as indicated on the plans shall be utilized to control livestock movements while allowing for contractor access. Extensive coordination with the landowner will be required. Access to the site will remain open and usable at all times.
c. Gravel Access Road, Haul Road, Existing Tomes Driveway Rehabilitation, and Gould Farm Road CR23/3 Rehabilitation improvements are to be maintained during construction and improvements are to remain following construction.
d. Topsoil in the LOD will be stockpiled separately from other excavations and re-used to cover the site at final grading.
e. Seasonal tree clearing restrictions apply. Trees within LOD must be felled between November 15, 2026 and March 31, 2027.
f. Trees over 8” diameter shall be saw cut prior to grubbing and removed from the site for disposal. All other organic material shall be burned on site if permits are acquired by the contractor. See Section 4 of the Specifications for full details.
g. Fencing Requirements:
i. Temporary Fencing: the temporary fencing requirements have been spelled out in the Specifications and the Plan details. This temporary fencing shall be electrified by the landowner. The Contractor is not required to electrify the fence but is required to coordinate with the landowner when the fencing needs moved or de-electrified throughout construction.
ii. Livestock Fencing: The livestock fencing specifications are found in an attachment within the Specifications. The Contractor is required to adhere to these requirements and to bid accordingly.
Two errors were identified in the Specifications.
Section 7.0 Drainage Structures, 7.2 Materials: No specifications were listed for the Filter Fabric for the Channel Energy Dissipator. This Fabric shall meet the requirements of AASHTO M 288, Section 8, Class 1, such as Mirafi 180N or approved equal.
Section 8.0 Unclassified Excavation, 8.4 Materials: Stone for the Bonding Bench, Toe-key Foundation, 2ft X 4ft, and 3ft X 3ft Underdrains is listed in the Specifications as AASHTO No
57. This should be AASHTO No 3 as shown in the Geotechnical Details and within the Geotechnical report.
These corrections shall be made as part of an addendum issued along with the answers to any technical questions asked today and submitted by October 20, 2026 to DEPProcurement@wv.gov.

5% of the TOTAL AMOUNT BID for the project.
2.0 Construction Layout
2.1 Description
This item consists of furnishing, placing, and maintaining construction layout stakes, (based on the original baselines and/or control points in the plans), necessary for the proper performance of the work under this contract including borrow/waste areas.
Construction limits must be clearly flagged, and Pre-Construction cross sections must be provided and approved prior to any disturbance or “unclassified excavation”. It shall further consist of determining the exact units of measure for payment. It also consists of checking and making any field adjustment to the plan alignment, grades and elevations as considered necessary by the Engineer.
The Contractor shall use competent personnel and suitable equipment for the layout work required and shall provide that it be done under the supervision of, or directed by, a
Professional Surveyor registered in the State of West Virginia with survey grade instruments that will provide real time sub-centimeter accuracy based on the project control provided in the Plans. Survey stakeout tolerances will be stated on plans; in absence of this, tolerances will be determined in the following way:
A survey plan will be submitted in writing or electronically to the Engineer. The submission will include a comprehensive list of survey personnel qualifications, the survey equipment to be used, and a detailed outline of the means, methods, and precision for tasks such as stakeout procedures, quantity measurements, volumetric confirmation, and as-built procedures. Approval of the plan is required prior to any work being performed.
Additionally, this item shall also include the preparation of “As-Built” Plans, including the Reclamation Plan and any others specifically requested by the State.
This item also includes taking photographs (with a date / time stamp) and video (where the date and time are indicated) of the entire site – including the road and adjacent structures – before and after construction. Photos and video are to be provided to the
State for record, in particular to show that damage to adjacent structure(s) or road(s) did not occur during the course of the Project.
This item will be paid according to the following schedule: 25% confirmation of cross-sections, 35% for project stakeout throughout construction, and 40% on the acceptance of as-builts. This layout shall include location stakes and cut-fill lines marked on the stakes for each pre-manufactured or formed on site structures (manholes, drop inlets, headwalls, etc.).
2.2 Materials
Conventional survey stakes, hubs, batter boards, templates, straightedges, and other devices necessary for laying out all parts of the work. Due to the presence of livestock, no flagging or tape may be used for staking.
2.3 Construction Methods
Construction Layout
The Contractor shall be responsible for the proper layout of the Work, including but not limited to: property boundaries, limits of disturbance, construction baselines, erosion and sediment controls, drainage structures, and/or feature identification for control of the work. Field markers must include the designated feature, appropriate stationing, and the cut/fill requirements to achieve final grade.
The Contractor shall make all calculations involved and shall furnish and place all layout stakes. The Contractor is responsible for verifying all surveying, measurements, estimates, or any other information provided by the State for accuracy. This layout shall include location stakes and cut-fill lines marked on the stakes for each pre-manufactured or formed on site structures (manholes, drop inlets, headwalls, etc). The layout shall also include centerline stakes with cut-fill lines for each channel and any areas of excavation and re-grade. The certified layout and cross-sections surveyed must be submitted to the
Engineer for approval prior to beginning construction for each item. Work shall be performed to the satisfaction of the Engineer to be considered complete.
The Engineer will provide the Contractor with survey information regarding the baselines and the existing surface features shown on the Plans. The Surveyor will field-verify the mapping by surveyed sections coincident to the baseline and surveyed profiles provided on the Plans. The Surveyor will field verify the baseline sections per the design plans and channel cross-sections at 200-ft intervals with a minimum of two (2) cross sections per channel. The cross-section comparison and a statement of general conformance will be provided to the Engineer for review and approval prior to any excavation activities.
The Surveyor shall provide field notes and shall set all additional stakes as needed, such as offset stakes, reference point stakes, slope stakes, grade stakes, stakes for drainage, or other structures, supplementary benchmarks, and any other horizontal or vertical controls necessary to secure a correct layout of the Work, including the re-establishment of the survey and construction baselines (as necessary), as shown on the
Plans.
The Surveyor shall be responsible for assuring the layout staking work is in conformance to the lines, grades, elevations, dimensions, and locations shown on the
Plans, or as required by the Engineer. The Contractor shall furnish a copy of their survey records for review by the Engineer and for the State’s permanent file. These records shall be furnished as they are completed during the progress of the Work.
Any inspection or review of the Surveyor's layout by the Engineer and the acceptance of all or any part of it shall not relieve the Contractor of their responsibility to secure the proper dimensions, grades, and elevations of the Work.
As the survey is completed, the Surveyor shall certify that the site layout is complete;
this includes, at a minimum, all necessary grades (both cut and fill), location and grades of all ditches, and all cross-sections related to cut and fill where grade lines are necessary for construction and or pay applications.
Survey Control
All survey control points must be clearly identified within the drawing and plans, labeled with their respective coordinates and elevations. The Surveyor is required to tie all project survey work to the baseline control provided by the Engineer or State. In the absence of furnished control points, the Surveyor shall establish, verify, and utilize new survey control.
The Surveyor shall gather field data utilizing the same coordinate system and elevation datum specified in the Plans. If field data is collected using an alternative geodetic datum, the Surveyor shall document the source datum and the translation parameters utilized to align the data with the Plans coordinate system and elevation datum.
The Surveyor shall be responsible for locating all property corner monumentation within the Limit of Disturbance (LOD) and provide coordinates to the Engineer for verification.
Upon verification, the Contractor shall remove and preserve disturbed monumentation, and reset original monuments to their location upon project completion. The Contractor shall exercise care in the preservation of stakes, monuments, and benchmarks, including existing property line markers, and shall have them reset at their expense if any are damaged, lost, displaced, or removed.
In the event site control is disturbed during construction, the Surveyor shall immediately reset and verify the control points for accuracy.
As-Built Plans
All “As-Built” Plans shall be provided to the Engineer and approved prior to the Final invoicing. The purpose of As-Built plans are to be a record of verification of all work completed by the contractor. A list of applicable pay items the Surveyor is required to field verify is included in these Specifications. As-Built information and plans must be certified by a West Virginia Registered Professional Land Surveyor (Surveyor) as to completeness and accuracy of the plan submittal (see certification requirement herein).
The As-Builts shall be provided in two forms of electronic format in PDF and AutoCAD
2020 or approved later version on either CD-ROM, DVD, or USB memory stick and one paper copy of the plans with the same size and scale as contained on the original
Plans. All surveyed locations, dimensions, and elevations shall be shown to the same precision as the Plans.
Additional information requested by the State, Engineer, or otherwise required by the
Contract Documents shall also be included in the record drawings.
All surveyed locations, dimensions, elevations, and quantities shall be shown to the same precision as the Plans unless otherwise approved by the Engineer. All labels will reference the exact item number and Name as listed in the bid summary table.
The Surveyor shall update the Cover Sheet by adding “AS-BUILT” to the Project Title, add current submission date and Revision Block and clearly identify in true color all sheets containing As-Built modifications or information in the index. Final As-Built cover sheet must include seal and signature of a Registered West Virginia Professional
Land Surveyor along with the following certification statement:
“I hereby certify that the information contained in this record drawing provides an accurate and complete representation of completed construction based upon field information obtained under the direct supervision of a West Virginia Registered
Professional Land Surveyor or otherwise verified quantities (i.e.: material ticket) provided by the Inspector or Engineer.”
The title block of each As-Built plan sheet must be updated to denote “As-Built” in the sheet description, current submission date, firm credentials and initials, and revision information if applicable.
As-Built drawings shall utilize a screened version of the final Plans as the base template to ensure original sheet layout, indexing, and spatial organization is maintained without modification. As-Built plans shall only include the sheets where information has been updated and field verified. Original design elements, including linework and annotations, must be screened in gray for clarity. All As-Built site features, contours, subsurface features, LOD and/or Contractor Work Limits (CWL) linework and labels
– shall be rendered in true color to ensure a distinct visual contrast between the design intent and final conditions.
The Surveyor shall provide an As-Built Legend to clearly define all specific As-Built features, symbology, and linework. At the Surveyor’s discretion, this may be configured as a comprehensive master legend in a single designated location or as individual, sheet-specific legends on each plan sheet.
Text and annotations shall note beginning and ending of work limits, including total linear footage, and be positioned to minimize the obscuring of As-Built data, free of overlapping text or linework. A background mask shall be applied to any annotations to ensure maximum legibility in areas where linework overlap cannot be avoided.
In the event that supplemental sheets are necessary, they shall be appended in sequential order and designated using an alpha character format relative to their corresponding baseline sheet (e.g., Sheet 12A).
Where final As-Built alignments deviate from the original design, the Surveyor shall include the original Plan profile sheets with the superseded profiles clearly crossed out in a distinct, true-color strikeout. Supplemental sheets depicting the As-Built profiles and/or cross sections shall be provided immediately following the original sheets.
All As-Built labeling shall maintain identical feature naming designations as established in the Plans, unless components have changed from design.
As-Built profiles and cross-sections shall be overlaid on the original construction design sheets to show verified existing ground topography, finalized As-Built grades with slope labels, contractor excavation cut/fill limits with area of section labeled, the location layout, invert elevations, and labeling of all installed drainage structures.
All subsurface or obscured features – including but not limited to seep collection, underdrains, toe keys, bonding benches, and related drainage structures that become inaccessible following backfill or burial – shall be surveyed for exact horizontal and vertical location prior to concealment and accurately shown on the final As-Built plans.
The Surveyor shall field-verify the installation of all temporary erosion and sediment control (E&S) features, including but not limited to: silt fence, safety fence, temporarily relocated fencing and gates, Stabilized Construction Entrances (SCEs), temporary access roads, laydown areas, diversion channels, check dams, riprap outlet protection, and sediment basins, shall be documented.
As-Builts shall include surveyed locations and all applicable horizontal and vertical dimensions of installed permanent improvements, including invert elevations where applicable.
For each installed component, including but not limited to pipes, conduits, drainage structures, manholes drop inlets, channels, splash pads, mine seals, subsidence fill areas, guardrail, and other appurtenances, the record drawings shall identify the type, size, length, material (where applicable), and all relevant rim, invert, and control elevations.
Surveyed boundaries of all waste and borrow sites, LOD and vegetative limits shall be shown and clearly labeled.
The Bid Item Schedule shall be modified to include an additional column designated
“As-Built Quantity”. All final quantities shall be recorded in a true-color format, utilizing the identical units of measurement established in the Plans. The final schedule totals shall correlate directly with, and verify, the quantities documented on the individual drawing sheets. For all other spreadsheets and tabular data within the plan set, columns may be modified to ensure adequate room to accommodate the recorded as-built information in true-color format with a line strike through design values.
Volumetric tables for As-Built unclassified excavation must be provided to validate and confirm all cross-sectional earthwork quantities.
Revegetation paid by the acre shall be based on As-Built plan-view acreage and isolated on a dedicated AutoCAD layer. All non-vegetated areas – including but not limited to:
graveled surfaces, pavement, roads, ponds, treatment infrastructure, un-vegetated channels, and buildings – must be excluded from both the acreage calculations and the associated CAD layer.
