About this file
Addendum No. 3 for IFB 250401 is issued by the City of Leesburg for the Construction of John L. Johnson Park Restroom Project. The addendum addresses multiple requests for information (RFIs) from potential bidders, providing clarifications on various aspects of the project including metal roofing panels (specifically MasterLok-FS), porch ceiling materials (AZEK Beadboard), interior finishes, and site restoration requirements. Key modifications include removing the 40-amp circuit for the lift station, eliminating the requirement for a List of Equipment, and confirming the need for Temporary Traffic Control (TCC) during sanitary sewer and water main tie-ins.
The addendum provides specific technical details for bidders, such as confirming a light broom finish for the concrete porch, specifying that interior block walls will have a 1/8" cementitious finish with smooth rubbed grout joints and ceramic tile up to 60" high, and noting that turf in disturbed areas must be restored. Additional clarifications include no irrigation system installation and leaving some details to be determined later, such as the color of the epoxy ceramic carpet resin for bathroom interiors. Bidders are required to acknowledge receipt of this addendum by signing and returning the provided acknowledgment form.

Q1. The Electrical Power plan shows a 40-amp circuit for the Lift Station however, per the Civil Drawings, it appears that the Sanitary Sewer is all gravity. Please confirm if this 40 amp circuit is required.
A1. There will be no circuit for lift station, please remove the circuit.
Q2. My question regarding the list of equipment appears to have been misunderstood. The IFB already contains two copies of the List of Supervisory Personnel but is missing the List of Equipment. What we need is the missing equipment list, not another supervisory personnel form. Please and thank you.
A2. This requirement is no longer required.
Q3. Given that this project does not appear to have any traffic maintenance associated with it, is the following line from the solicitation still a necessary requirement or was it standard language included from past ones: "Bidder must submit the name and proof of either one (1) employee or a subcontractor that maintains a State of Florida MOT certificate and capable of producing MOT plans for the identified projects. The employee or subcontractor shall have the experience for the establishment of Maintenance of Traffic on FDOT right-of-way within the last three (3) years."
A3. Temporary Traffic Control (TCC) formerly know as MOT will be needed for the tie in of the sanitary sewer and water main.
