About this file

Addendum 2 for the East and West Forks Project – Bridge Replacement (Project 2018-10a) is issued by the City of Ashland, modifying several key aspects of the original bid documents. The addendum changes the bid due date from April 22nd to April 24th, with all other bid opening parameters remaining unchanged. The scope of work is expanded to include the removal of 6 trees near the West Fork of Ashland Creek, which were identified during the bid walk as interfering with crane lifts for bridge installation. The trees will be removed from the direct bridge area and spread out on the forest floor to match existing fallen trees, with no lumber sale permitted.

The addendum also shifts the responsibility for obtaining project permits from the Contractor to the City of Ashland. Additionally, it introduces comprehensive environmental protection guidelines for equipment, vehicles, and power tools, emphasizing the critical nature of the Ashland Creek watershed and Reeder Reservoir to the city's water supply. The guidelines include strict protocols for equipment cleaning, fluid leak prevention, operation near water bodies, and immediate response to potential environmental contamination. A final provision requires the Contractor to maintain and restore access roads to pre-construction conditions, ensuring minimal environmental impact throughout the project.

Addendum 2

East and West Forks Project – Bridge Replacement Project 2018-10a

1) The bid due date for the East and West Forks Project – Bridge Replacement (Project 2018- 10a) is changed from April 22nd to April 24th . The location of bid opening, the time of day of bid opening and the methods of delivery all remain the same.

2) The scope of work is modified to add the removal of 6 trees located near the West Fork of Ashland Creek to bid schedule item 50 “Removal of Structures and Obstructions”. These trees were identified during the bid walk as interfering with the crane lifts required to install the bridge. Trees shall be removed from the direct area of the bridge and spread out on the forest floor to match existing fallen trees. Trees can be cut into sections to facilitate this process. Lumber sale is not allowed.

3) The City of Ashland will be responsible for obtaining the permits required for the construction and installation of the project scope from the appropriate government agencies. This addendum supersedes language in the ITB and contract stating such permitting to be the responsibility of Contractor.

4) In Exhibit N, Sheet “General Notes” add the following section

EQUIPMENT, VEHICLES AND POWER TOOLS

1. The Ashland Creek watershed and Reeder Reservoir are critical components of the City’s water supply. Through the course of their work, the Contractor’s top priority shall be protection of the Ashland Creek watershed and its waters, and the Contractor shall take all reasonable measures to protect water quality and the environment. Protection includes, but is not limited to, preventing any deleterious substance from contacting any water body, preventing the introduction of non-native plant species, preventing compaction of pervious soils, and limiting disturbance to the natural environment.

2. Select, operate and maintain all heavy equipment, vehicles and power tools to minimize damage to natural vegetation and compaction of permeable soils, e.g. low-pressure tires, minimal hard-turn paths for track vehicles, use of temporary mats or plates to protect wet soils.

3. No equipment fueling or repair shall be allowed within 150 feet of any water body, unless express permission is provided by the Owner in writing if an Owner is not onsite, and verbally if the Owner is onsite and witnessing the fueling or repair activities.

4. Before entering any area within 150 feet of a water body.

4.1. Power wash all heavy equipment, vehicles and power tools, allow them to fully dry and inspect them for fluid leaks and to make certain no plants, soil, or other organic material are adhering to the surface.

4.2. Ensure all equipment to be operated within 150 feet of any water body is leak free and operating with biodegradable products wherever possible.

5. Inspect and repeat cleaning as often as necessary during operation to keep all equipment, vehicles and power tools free of external fluids and grease, and to prevent a leak or spill.

6. No use of heavy equipment, vehicles or power tools below OHW will be allowed unless permission in writing is granted by the Owner.

7. If personnel must enter the water, inspect any boots, waders or other gear/equipment to be used in or near water and remove any plants, soil, or other organic material adhering to the surface.

8. Ensure that any generator, crane or other stationary heavy equipment that is operated, maintained or stored within 150 feet of any water body is also protected as necessary to prevent any leak or spill from entering the water.

9. If, during the course of work, a piece of equipment is found to be leaking, it shall immediately be moved away from the water body, and the first order of work shall be to contain any leaking substances and prevent contact with the environment. The second order of work shall be to clean up any spill to the extent that leaked substances are fully eliminated from the environment. The leaking equipment may be repaired onsite if the repair can be made without any risk of contamination of the environment or waters. Otherwise, the leaking piece of equipment shall be expeditiously removed from the site once the leak is stopped

10. The Owner may at any time require that the Contractor to stop work, if in the opinion of the Owner the Contractor’s equipment or actions pose an unnecessary risk to the environment.

5) In Exhibit N, Sheet “General Notes” add the following two sentences to item (1) in section SITE following the first sentence which reads “Existing dirt/gravel access roads may be utilized by contractor to gain access to the project sites.”

Contractor shall be responsible for, and shall take reasonable measures to preserve access road integrity. Should the dirt/gravel access roads become degraded through the course of the Work, Contractor shall restore them to pre-construction conditions.
