About this file
This is an addendum to a Request for Quotes (RFQ) issued by the West Virginia Department of Commerce, Regulation, and Development for wastewater sampling, reporting, and inspection services at the North Central Regional Jail (NCRJ) in West Union, West Virginia. The solicitation number is ARFQ 0608 DCR2300000132. The primary service requirement involves the vendor purchasing, setting up, and maintaining a composite autosampler that meets the specifications outlined in WV/NPDES Permit No. WV0020109 to collect wastewater samples exiting the NCRJ's pretreatment system before it enters the Town of West Union's collection system. The addendum specifically modifies Section 4.1.1.1 to clarify that the vendor, rather than the agency, is responsible for providing and maintaining the composite sampler for the entire contract term. The vendor questions and agency responses document confirms that vendors must independently identify the appropriate autosampler model and specifications based on permit requirements, as the agency will not specify or provide the equipment or its manual.
The addendum establishes that all maintenance, malfunctions, and repair responsibilities for the autosampler rest solely with the vendor throughout the contract period. Vendors are required to acknowledge receipt of all addenda issued for this solicitation by completing and submitting the Addendum Acknowledgment Form with their bid proposal, as failure to do so may result in bid disqualification. The addendum was revised on June 8, 2012, with subsequent updates on October 22, 2020, to the acknowledgment form itself.
