About this file

This is an Addendum No. 1 cover document issued by the City of DeBary for the Small Scale Stormwater Improvement Project No. 5, Bid No. 12-19. The addendum modifies the original bid documents and was released on October 16, 2019, with a bid date of October 22, 2019, at 10:00 AM. The project involves stormwater drainage improvements on Floridana Road, including the installation of stormwater pipes in the area between residences at 132 and 134 Floridana Road, with impacts to wetland areas. Key modifications include revisions to the bid form, where Pay Item 101-1 (Mobilization) now includes the cost of providing Payment and Performance Bonds, and Pay Item 990-4 (CCTV Inspection of Installed Stormwater Pipes) has been deleted as the City will conduct inspections using its own equipment. The addendum addresses critical project conditions, including permitting, easement acquisition, and utility coordination requirements. Road closure is permitted for a continuous period not to exceed three weeks, with provisions for emergency access between 7 PM and 7 AM. The Engineer's Estimate for the project is $153,196.40.

The addendum includes twelve bidder questions and corresponding answers addressing insurance requirements, road closure restrictions, wetland construction impacts, permits, drainage easement agreements, engineer's estimate confirmation, utility relocation coordination, and pay item specifications. The St. Johns River Water Management District issued permit exemption #157435-1 dated May 7, 2019, authorizing the work with specific delineated construction limits. Recorded drainage easement agreements with property owners at both addresses and a Geotechnical Engineering Report dated December 4, 2018, are included with the addendum documents. The City confirmed that Duke Energy power pole guy wire relocation is outside the contractor's scope, with contract time extensions available if project delays result from utility work. The bid documents are based on Florida Department of Transportation standards, with measurement and payment addressed in Special Provisions Article 27.

Questions from Bidders

Question 1.

Specs. Indicate that Owners Protective Liability Coverage is required. This is an outdated form of insurance to cover owner’s liability on a project. However, being an additional insured on the policy replaces that needed coverage. Can this be removed from the Bid Documents?

Answer to Question 1.

No, the insurance requirements remain unchanged.

Question 2. Will there be any restrictions to a road closure? Roadway is approximately 18” wide and there will be no way to install storm drainage at depth shown AND provide a single lane of traffic. Please advise.

Answer to Question 2.

Within the project limits, Floridana Road may be closed to all thru traffic for a continuous period not to exceed a maximum of three (3) weeks. During the three (3) week period of continuous road closure, the Contractor shall provide and maintain vehicle access to the driveway of each residence within the road closure area for the resident’s vehicles and for potential emergency responders during the hours of 7 PM to 7 AM.

Question 3.

Will there be any restrictions for construction that impacts the wetland area (temporary or permanent) as shown on Sheet 10?

Answer to Question 3.

The St. John’s River Water Management District (SJRWMD) issued a permit exemption (#157435-1 dated May 7, 2019) to the City of DeBary based on the information presented on Sheet 10 of the construction plans. Complying with the permit exemption requires that all construction related impacts to occur within the limits that are delineated on the construction plans.

Question 4.

Has City obtained all necessary permits for construction within the wetland area? Can copies be provided?

Answer to Question 4.

A copy of the SJRWMD permit exemption (#157435-1 dated May 7, 2019) that was issued to the City of DeBary is included with the Addendum No. 1 – Bid Documents.

Question 5. Will the use of a trench box be allowed for the construction in the area of “special requirement for construction activities between the houses at 132/134 Floridana Road”?

Answer to Question 5.

Yes, if the Contractor determines that a trench box is sufficient support for the required shoring, a trench box may be used for the construction in the area of “special requirement for construction activities between the houses at 132/134 Floridana Road.

Question 6. Has City obtained 20’ proposed drainage easement from homeowners? Can you provide agreement?

Answer to Question 6.

Copies of the recorded Drainage Easement agreement between the City of DeBary and the Owner of 132 Floridana Road and the Owner of 134 Floridana Road are included in the Addendum No. 1, Bid Documents.

Question 7. Please confirm the Engineer’s Estimate is $153,000 as stated at the pre-bid.

Answer to Question 7.

The Engineer’s Estimate is $153,196.40.

Question 8. Sheet 7 indicates that power pole guy wires are to be relocated. Will this be completed prior to NTP?

Answer to Question 8.

The date of NTP and the date of the work completion by Duke Energy cannot be determined at this time.

The City of DeBary will issue an extension of the Contract time if the project completion is delayed by the Duke Energy relocation of the power pole guy wires.

Question 9. Plans do not address power pole. Is it to be relocated by others? Sheet 7 also shows power pole to be within limits of thickened edge of sidewalk. Please advise.

Answer to Question 9.

The existing power pole will not be relocated. Although the final sidewalk alignment will be coordinated in the field with the Owner’s Representative, the proposed sidewalk can be narrowed slightly if necessary to avoid the existing power pole.

Question 10.

Note at end of bid form states “pay items obtained from Florida Department of Transportation (FDOT) Basis of Estimates (BOE) and as modified by City”. Please indicate which pay items were modified by the City and please provide measurement and payment for each.

Answer to Question 10.

Pay Items included on the Bid Form – Addendum No. 1 generally follow the FDOT – Basis of Estimates. However, the Work required under the Pay Items listed on the Bid Form – Addendum No. 1 is shown on the Construction Drawings and quantities are provided on the Bid Form – Addendum No. 1. Measurement and Payment for Unit Price Items is addressed in Special Provisions Article 27.

Question 11.

As an example to Question #10, pay item 1050-1 is not found in FDOT BOE. Please clarify what is to be included in this item and how we are to be paid for it.
