About this file

This document is a Supplemental Conditions and CDBG Required Forms document from the City of New Smyrna Beach for the Pettis Park Pavilion Project, identified by Project ID ITB 19-25-CIP. The document outlines specific requirements for contractors and subcontractors who may be selected for this federally assisted project. If chosen, the contractor must submit four specific forms at or before the preconstruction meeting: a Minority and Women Business Reporting Form, Section 3 Business Concern Certification, Section 3 Project Workforce documentation, and a Section 3 Certification.

The document is a standard procurement form that requires the potential contractor to review supplemental conditions for federally assisted projects and provide company information, including the company name, an authorized officer's name and title, their signature, and the date. While no specific pricing or funding details are provided in this document, it indicates that the project is likely funded through Community Development Block Grant (CDBG) sources and requires specific documentation to ensure compliance with federal guidelines related to minority business participation and workforce requirements.
