Supplemental Conditions and CDBG Required Forms

Title of Project Name: Pettis Park Pavilion Project ID: ITB 19-25-CIP

The below CDBG forms are provided in the Attachment Section of the above-mentioned Request for Proposal (RFP) Solicitation document.

If chosen as the contractor, you hereby acknowledge, the following items are required to be submitted to the City of New Smyrna Beach at or before the preconstruction meeting by the Prime Contractor and Subcontractors:

A. Minority and Women Business Reporting Form

B. Section 3 Business Concern Certification

C. Section 3 Project Workforce

D. Section 3 Certification

Proposers are encouraged to review all supplemental conditions for federally assisted projects under the attachments category.
