About this file
This is an Acceptance of Offer Form issued by the City of Phoenix, Arizona, for a municipal contract award. The document formalizes the City's acceptance of a contractor's sealed offer for a five-year Portland Cement (Bulk) supply contract for the Street Transportation Department. The procurement solicitation (IFB-26-0033A) requires the delivery of approximately 300 tons annually of ASTM C-150, Type II Low Alkali cement to the Street Transportation Preventive Maintenance facility in Phoenix. Deliveries must occur between 7:00 am and 2:00 pm, Monday through Friday, excluding city holidays. The solicitation timeline included a non-mandatory pre-offer conference on November 4, 2025, written inquiries due by November 19, 2025, and final offer submissions due on November 24, 2025, at 2:00 pm local time. The contract is scheduled to commence on July 1, 2026, or upon City Council award and City Clerk recording, whichever is later. The contractor is cautioned not to commence any billable work or provide materials or services until receiving purchase order or contract documentation from the City.
Pricing will be firm and fixed for the initial two years, with potential annual price adjustments thereafter subject to written documentation and approval by the Chief Procurement Officer. The procurement emphasizes significant Disadvantaged Business Enterprise (DBE) and Small Business participation, requiring contractors to submit annual participation plans, maintain detailed documentation of DBE engagement efforts, and report monthly through the City's Certification and Compliance System. Contractors must demonstrate good faith efforts to identify and engage DBE-certified suppliers and utilize federal and city databases to locate opportunities. The contract includes requirements for submission of a Certification of Full/Final Payments to DBE Firms form and potential requests for DBE subcontractor substitutions or exemptions. No specific contract value is explicitly stated, though the annual requirement of 300 tons suggests a multi-year procurement with significant economic impact. The form was approved as to form by the City Attorney on January 19, 2017, and is on file with the City Clerk.
