About this file
This is an Acceptance of Offer Form issued by the City of Phoenix, a municipal corporation in Arizona, for contract execution and binding authorization. The form serves as formal acceptance of a contractor's offer to provide materials or services as specified in the attached contract and related solicitation documents. The document confirms that the contractor is now bound to provide the listed materials or services according to all terms, conditions, specifications, amendments, and the accepted offer. The form includes signature blocks for the City Manager, a designated Director or delegate, and the City Clerk, with an attestation date in 2025. The form explicitly cautions contractors not to commence any billable work or provide materials or services until receiving official purchase order or contract documentation from the City.
The Acceptance of Offer Form references an underlying Event Equipment Rental Services solicitation (PKS-IFB-26-0175) issued by the City of Phoenix Parks and Recreation Department for a five-year term commencing April 1, 2026. The solicitation seeks a contractor to provide comprehensive equipment rental services across 150 line items including seating, tables, tents, canopies, staging, audio-visual equipment, lighting, power distribution, crowd control accessories, and climate control equipment. Key deadlines include a non-mandatory pre-offer conference on January 6, 2026, a written inquiry deadline of January 9, 2026, and sealed offer submissions due January 26, 2026, at 2:00 PM. The award will be made to the lowest responsive and responsible bidder. Contractors must maintain insurance coverage including $2,000,000 in General Liability, $1,000,000 in Automobile Liability, and statutory Workers' Compensation, with pricing firm and fixed for the initial year and annual increase requests permitted thereafter. Liquidated damages of $2,000 per calendar day apply for delays in equipment services, and contractors must guarantee delivery within two working days for standard purchases and four hours for emergency services with no additional delivery fees.
