The form applies to IFB #2025-13 for Platform Holly Caretaker Maintenance & Oversight Services, a public works project managed by the California State Lands Commission. Platform Holly is an offshore oil and gas platform located in the Santa Barbara Channel, approximately two miles southwest of Coal Oil Point in Santa Barbara County. The platform, installed in 1965 and shut-in following the 2015 Refugio Oil spill, has been under state control since 2017. The contract calls for caretaker services involving routine platform visits and remote monitoring to prevent trespass and ensure navigation safety systems remain operational following the platform's hardening phase, which is scheduled for completion by Fall 2024. Bids are due February 11, 2026.

Any company with business activities or operations outside the United States within the previous three years must complete this certification form to be eligible to bid. Bidders must certify under penalty of perjury that they are either not a scrutinized company as defined in Public Contract Code Section 10476, or alternatively, provide written permission from the DGS Director authorizing a scrutinized company to submit a proposal on a case-by-case basis if deemed in the state's best interest. Companies with no international business activities within the three-year period are exempt from this requirement. The form requires the authorized company representative to provide the company name, Federal ID Number, signature, printed name, and title.
