Quality Assurance Surveillance Plan (QASP)
For: Spectrum Guard Pro Warranty Support Service, Marine Forces Reserve
1.0 Introduction
This Quality Assurance Surveillance Plan (QASP) is a Government document designed to ensure the contractor’s performance meets the standards set forth in the Performance Work Statement (PWS). This plan details the methods the Government will use to monitor, evaluate, and document the contractor's performance. As stated in PWS Section 1.5.1, this plan defines how performance standards will be applied, the frequency of surveillance, and the acceptable performance levels. It is not a substitute for the PWS; the PWS remains the authoritative document for all requirements.
2.0 Purpose
The primary purpose of this QASP is to provide a structured approach for the surveillance of the contractor’s performance to ensure services conform to the PWS requirements. It aims to:
• Define the roles and responsibilities for inspection and evaluation.
• Outline the key performance objectives and their required quality standards.
• Specify the methods of surveillance to be used for monitoring performance.
• Establish clear, measurable acceptable quality levels (AQLs) for assessing performance.
3.0 Performance Requirements & Surveillance Matrix
The following table details the performance objectives, standards from the PWS, surveillance methods, and acceptable quality levels the Government will use to measure contractor performance.
Performance Objective
Performance Standard (PWS
Reference)
Method of Surveillance
Acceptable Quality Level (AQL)
1. Technical Support & Responsiveness
Adhere to the Service Level Agreements (SLAs) for all reported issues.
Provide an initial response for Level 1, 2, and 3 issues within the specified timeframes. (4.0)
100% Inspection of Help Desk & RMA Logs: The COR will review the Monthly Warranty and Maintenance Report and may directly inspect ticket logs to validate response times for all reported issues.
100% of initial responses must meet the timeframes specified in the SLA table (e.g., within 4 business hours for Level 1). No exceptions.
2. Hardware & Software Resolution
Resolve issues and complete RMA turnarounds within the timelines specified in the SLA table. (4.0, 5.0)
100% Inspection of RMA & Ticket Logs: The COR will track resolution and shipping dates via the Monthly Report to ensure compliance
98% of all hardware/software issues are resolved or have replacement items shipped within the required turnaround time for their severity level.
with SLA resolution timelines.
3. Software & Firmware Maintenance
Provide all bug fixes, security patches, minor version updates, and major version upgrades as they become commercially available. (3.0)
Periodic Inspection & Review of Monthly Report:
The COR will review the summary of patches/updates in the monthly report and periodically validate that systems have access to the latest software versions.
100% of required software/firmware updates and patches are made available to the Government.
4. Logistics & RMA Process
Issue an RMA number within one
(1) business day for validated Level 1 or 2 hardware failures.
Cover all shipping costs for warranty repairs. (5.0)
100% Inspection of RMA Logs: The COR will review the RMA issuance dates in the monthly report and verify that shipping costs are not charged to the Government.
100% of RMAs are issued within the one-business-day timeframe. Zero shipping costs invoiced to the Government for warranty repairs.
5. Monthly Reporting
Submit a Monthly Warranty and Maintenance Report that is accurate, complete, and contains all required information (ticket counts, RMA statuses, software updates). (6.0)
100% Inspection:
The COR will review each monthly report for timeliness, completeness, and accuracy.
Reports must be submitted by the 5th business day of the following month. No more than one (1) report requiring correction due to inaccuracies per quarter.
4.0 Methods of Surveillance
The following methods will be used by the Government to monitor the contractor's performance:
• 100% Inspection: Used for critical tasks where full compliance is expected. This method will be applied to all deliverables (especially the Monthly Report) and to tracking all SLA response times and RMA issuances.
• Periodic Inspection: The COR will conduct surveillance at scheduled or unscheduled intervals to check the status of ongoing repairs, software availability, and overall contractor performance.
• Review of Reports: The primary method of surveillance will be the detailed review of the Monthly Warranty and Maintenance Report, which provides the data needed to verify SLA compliance and track all service actions.
5.0 Documenting Performance
The COR will document the results of all surveillance activities. This documentation will be the basis for verifying contractor invoices and will be retained in the official contract file. If performance is deemed unacceptable (i.e., fails to meet the AQL), the COR will notify the contractor and the Contracting Officer. The contractor will be required to provide a written explanation for the non-conformance and a corrective action plan to prevent recurrence.
