All work must comply with Colorado Division of Water Resources requirements and be performed by a licensed well contractor per Colorado Board of Examiners standards (2 CCR 402-14). The period of performance is 360 calendar days with all work to be completed within 90 days of starting (Monday through Friday, excluding federal holidays). Payment is structured in three line items covering demolition, well investigation and improvement, and water system installation. The contractor must comply with BLM wildlife environmental protection measures outlined in Appendix A, including noxious weed management, fire extinguisher and tool requirements on equipment, camping and worksite conditions, and hazardous materials handling.

TRFO Basin Well Improvement Performance Statement of Work

General Requirements

Basin Well Improvement

1 GENERAL REQUIREMENTS

1.1 GENERAL

The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform the work as defined in this Performance Work Statement.

1.2 PERIOD OF PERFORMANCE

The period of performance shall be 360 days. Contractor shall complete all work within 90 days of starting work. 360 days period of performance is provided to allow contractor to fit the work into their schedule. Work shall occur Monday thru Friday except Federal holidays.

1.3 QUALITY CONTROL

The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this performance statement of work. The contractor’s quality control program is the means by which the Contractor assures that the work complies with the requirements of the contract.

1.4 QUALITY ASSURANCE

The Government will assure quality of the contractor’s work.

1.5 POST AWARD CONFERENCE

The Contractor shall attend a post award, on site meeting, prior to starting work. The purpose of the meeting is to:

• Introductions.

• Site familiarization tour and verify existing conditions.

• Review and discussion of project requirements and schedule.

1.6 CONTRACTING OFFICER’S REPRESENTATIVE (COR)

The (COR) will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

Project Specific Requirements

2 PROJECT SPECIFIC REQUIREMENTS

2.1 SUMMARY OF WORK

Contractor shall provide a turnkey solar powered, well water system that automatically provides and maintains water to a wildlife drinking tank with minimal human input except winterization and annual maintenance. System shall be designed to have a solar pumping system at an existing water well. Provide water to an elevated buried storage tank. The buried storage tank will gravity feed the drinking tank. An additional tee and valve shall be provided for a future upgrade for a stockwater tank. System shall be designed to be freeze proof and operational in the winter to the frost free hydrant at the drinking tank.

2.2 PROJECT LOCATION

Latitude: 38.06529167° N Longitude: -108.63154723° W From Naturita, CO. Travel south on CO-141 for approximately 12 miles. Turn right (west) on County Road U29. Travel approximately 5 miles. The project is on the left (south).

2.3 REFERENCE INFORMATION

See attached for existing well information.

See attached for existing condition pictures.

The BLM has not operated the well pump in at least 20 years.

2.4 EXISTING ITEMS TO DEMOLISH

Existing items to demolish:

Well house (including all components associated with well house; cistern, loadout, foundation, utilities, piping, etc)

Stock tank.

Storage tank.

Well pump, if applicable.

The existing well and well casing shall be protected in place during demolition.

All materials demolished, shall become the property of the Contractor and shall be removed from Government property and disposed of in a legal manner.

2.5 GENERAL WORK REQUIREMENTS

All well work shall be in accordance with Colorado Division of Water Resources requirements.

Well contractor shall be licensed in accordance with Colorado Board of Examiners of Water Well Construction and Pump Installation Contractors, as outlined in the Colorado Revised Statutes and the Board’s rules (2 CCR 402-14)

All work shall be in accordance with Code of Colorado Regulations: 2 CCR 402-2.

2.6 WATER WELL INVESTIGATION AND IMPROVEMENT

Investigate existing conditions of the well and well casing.

Pump test the well.

o Verify the well is capable of producing water as required in WATER SYSTEM

REQUIREMENTS.

Notify the COR of any issues discovered during investigation that won’t allow completion of the work per this SOW.

Provide surface casing, well cap, and grout/well seal as needed.

o Extend steel surface casing above grade.

o Seal surface and well casings if needed to comply with CO DWR requirements.

o Prepare well casings for a pitless adapter (see Section 2.7.3) o Provide and install suitable well cap

Disinfect the well with chlorine solution (shock the well) per the requirements of 2 CCR 402-2 Rule 15

2.7 WATER SYSTEM

2.7.1 WATER SYSTEM DESIGN

Design a water system that provides 500 gallons of water per day to wildlife, delivered at the drinking tank.

Provide details (manufacturer, model, etc.) of all components (pump, solar panels, switches, tanks, etc.) installed.

2.7.2 SOLAR POWER

Provide a complete solar power system, as necessary, to power the water system.

Provide livestock exclusion fence, with man gate, to protect solar power system and well head from livestock.

2.7.3 WELL AND PUMP

Provide pump, sized appropriately for the water system.

Provide pitless adaptor compatible with an artesian well.

2.7.4 STORAGE TANK

Provide a new 5,000 gallon storage tank with the following features.

o Inlet and outlet.

o Vent.

o Overflow.

o Access riser.

o Rated for full bury depth as required.

Bury the storage tank to protect from freezing, per manufacturers installation requirements.

Provide a float switch or similar means of water level control in the storage tank.

Provide new livestock exclusion fence around storage tank.

o Contractor may reuse or repair existing fencing around existing storage tank after installation of new storage tank.

2.7.5 DRINKING TANK

Provide a 1,000 gallon drinking tank, approximate size 9’ diameter x 28” depth, with drain valve and escape ramp for wildlife.

Provide frost proof hydrant to fill the drinking tank.

Provide a wildlife friendly, livestock exclusion fence around the wildlife drinking tank, with man gate.

2.7.6 MISCELLANEOUS WATER SYSTEM COMPONENTS

Contractor shall provide any piping, floats, valves, switches, and other componets, as required, for a complete and functioning solar powered livestock and wildlife water system, including winterization and maintenance.

o Contractor not responsible for winterization and maintenance.

Water system shall maintain water within the top 6 inches of the drinking tank.

Water distribution pipes (between well and tanks) shall be buried a minimium of 36 inches.

Provide tee and curb stop with capped riser pipe for future addition of livestock drinking tank.

o Livestock drinking tank not required in this statement of work.

All provided water system components shall be new.

Contractor shall demonstrate proper functioning of the water system.

o Contractor shall train BLM operator on winterization and maintenance procedures.

2.8 PRICE AND PAYMENT

Payment items for the work of this contract for which contract payments will be made are listed in the SCHEDULE and described below. All costs for items of work, which are not specifically mentioned to be included in a particular job are included in the listed job item most closely associated with the work involved. The price and payment made for each item listed constitutes full compensation for furnishing all labor, materials, and equipment, and performing any associated Contractor quality control, environmental protection, meeting safety requirements, tests and reports, and for performing all work required for which separate payment is not otherwise provided.

2.8.1 DEMOLITION

Payment will be made for all costs associated with paragraph 2.4 EXISTING ITEMS TO DEMOLISH. Including transportation and disposal.

2.8.2 WATER WELL INVESTIGATION AND IMPROVEMENT

Payment will be made for all costs associated with paragraph 2.6 WATER WELL INVESTIGATION AND IMPROVEMENT, including sub-paragraphs.

2.8.3 WATER SYSTEM

Payment will be made for all costs associated with paragraph 2.7 WATER SYSTEM, including sub-paragraphs.

3 ADDITIONAL WORK REQUIREMENTS

The Contractor shall adhere to all conditions identified in Appendix A – BLM TRFO Wildlife Environmental Protection Measures.

Dear Mr. Kesterke:

Your March 28, 1988 request for an exception to Section 4( 3) of the rules and regulations for well construction for the above - referenced airstrip well, previously constructed and in use for livestock watering, has been reviewed.

The Board of Examiners of Water Well Construction and Pump Installation Contractors has granted your request to allow the well to be completed using

3. 5 - inch diameter casing which is compatable with your windmill pump connections pursuant to your March 28, 1988 request.

The Board is concerned with the fact that this well and several others in your District, former uranium exploration holes, were converted to water wells by unlicensed individuals. However, in light of the circumstances and the use of the wells being limited to livestock purposes, the Board waives the requirements of Section 3( 2) of their rules and regulations, requiring the work to be done by licensed contractors.

We are submitting the attached information in response to a " Request for Correction or Additional Information on Statement of Beneficial Use" submitted to our office on September 22, 1989 for the Basin Well, permit # 150834 and the Airstrip Well, permit # 150845. We have completed a signed and notarized Statement of Beneficial Use of Ground Water" and a signed " Well Completion and

Pump Installation Report" for each well. We are providing this letter as some background information.

Airstrip Well, permit # 150845: As your records probably show, the Airstrip Well is a converted uranium test bore that was completed as a water well for livestock use only. The well was completed with 3Y,' PVC casing for which a waiver was given by your office on March 30, 1988. We have provided all of the information we have on file regarding well completion and pump testing. We regret the catch up nature of these submittals and will continue our efforts to bring these wells into compliance with state regulations.

Basin Well. permit # 150834: Again, we have provided the information we have on file concerning the well completion and pump installation. Our records show that the Basin Well was drilled by Donald D. Jennings of the Atlas Water Well Drilling Company in May of 1972. We are trying to locate Mr. Jennings and will request a submittal of the proper forms to your office, if we can locate him and if he has still maintained these records. In the meantime, please accept the enclosed information as the information we have on file.

Appendix A – BLM TRFO Wildlife Environmental Protection Measures Noxious Weeds

• Vehicles and heavy equipment are one of the primary agents for the spread of noxious weed seed to public lands. In eƯorts to mitigate the spread of weed seed to BLM lands the following actions are required for contractors prior to transport to a BLM project area.

The Contractor will be responsible for power-washing or comparable cleaning, to ensure that noxious weed seeds are removed from all equipment and vehicles used on the project prior to entering the project area. BLM will require a pre-implementation inspection to ensure compliance which would be arranged to occur before the equipment enters BLM lands. The Contractor is responsible for any additional transport or expenses associated with non-compliance.

Support vehicles (pickups, fuel/service vehicles, transports, dump trucks, etc.)

Pressure wash radiator to flush seeds.

Pressure wash undercarriage to remove accumulations of mud and soil that may contain seeds.

Heavy Equipment (dozers, road graders, excavators, backhoes, loaders, etc.)

Pressure wash radiator(s) to flush seeds.

Pressure wash tracks to remove accumulations of soil.

Pressure wash all areas of soil/debris accumulations (i.e. steps) Pressure wash blades and buckets where soil/mud is accumulated.

Empty pre-cleaners (air intake) before transport.

Fire Extinguisher and Tools on Equipment

• While in use, each internal combustion engine including tractors, trucks, dozers, hydro-axes, welders, generators, stationary engines, or comparable powered equipment shall be provided with at least the following:

1. One fire extinguisher, at least 5#ABC with an Underwriters Laboratory (UL) rating of 3A - 40BC, or greater. Extinguisher shall be mounted so as to be readily available for use (not locked in a tool box or chained to a seat, for example)

2. One shovel, sharp, size “O” or larger, round-pointed with an overall length of at least 48 inches.

3. One axe, sharp, double bit 3½#, or one sharp pulaski.

• It is prohibited to operate or use any internal or external combustion engine, on any timber, brush, or grass covered land, including trails traversing such land, without a spark arrester maintained in eƯective working order and meeting guidelines set forth in National Wildfire Coordinating Group Publication NFES 1363 and 2363.

CAMPING AND WORKSITE CONDITIONS

The contractor or his employees, upon documented permission from the CO/COR, may establish temporary work camps on public lands administered by the Bureau of Land Management. Camps shall be kept tidy and free of refuse. These camps may consist of mobile trailers, pickup campers, tent trailers, or tents. The contractor will be required to rehabilitate the camping area to the satisfaction of the CO/COR. Camps are subject to all fire restrictions placed by state, county or federal authorities.

Hazardous Materials

Use of Hazardous Materials and/or petroleum products requires that all appropriate State and Federal Regulations be complied with including, but not limited to, Material Safety Data Sheets (MSDS) on hand and use of necessary Personal Protective Clothing (PPE).

On-site disposal of Hazardous Materials or Waste including hydrocarbons is not authorized. On-site disposal will subject the contractor to at least the cost of reclamation and the appropriate disposal of contaminated soil.

Incidental (de minimus) leaks from fittings, gaskets or ruptured hoses will not subject the contractor to remedial requirements. They will be considered to be normal and unavoidable losses. Continual leaks will be noted on inspection reports and correction through maintenance required.

Maintenance and repair operations that require the draining of engines or hydraulic systems may be conducted on site only if the fluids are captured, containerized, and removed from public lands for proper disposal.
