Statement of Work

KEAMS CANYON ELEMENTARY SCHOOL

Gym Floor Replacement and Repair Keams Canyon, AZ 86034

A. GENERAL

1. Purpose:

Keams Canyon Elementary school is seeking the services of a qualified contractor for the gym floor replacement and repair project located at Keams Canyon Elementary School located at 100 Main Street, Building 48 Keams Canyon, AZ, 86034. The selected Contractor shall be responsible for providing all necessary services, including but not limited to selective design, construction, installation, and supervision, in accordance with the requirements outlined in this statement of work, along with the contract terms, conditions, specifications, and any specific requirements.

The Contractor shall adhere to all relevant codes, including architectural, civil, structural, mechanical, electrical, plumbing, and other applicable codes as determined by BIA/BIE. All work performed must conform to the prescribed Codes and Standards, ensuring safety, quality, and compliance of the project.

The purpose of this Statement of Work (SOW) is to invite proposals from potential contractors for the work required at this location. It is important to note that this SOW does not constitute an award or a commitment to award the project. Instead, it serves as an initial step in the procurement process, allowing interested parties to submit their proposals for consideration. The final selection and award of the contract will be based on a thorough evaluation of the received proposals and compliance with all relevant procurement procedures and criteria.

2. Project Location:

Keams Canyon Elementary school is situated in the city of Keams Canyon, Arizona.

3. Definitions and Acronyms:

“ADAABAAG” means “Americans with Disabilities Act and Architectural Barriers Act Accessibility Guidelines”.

“A-E” refers to the Architectural and/or Engineering firm. “AHJ” refers to the Authority Having Jurisdiction “ASTM” refers to the American Society for Testing and Materials.

“BIA” refers to the Bureau of Indian Affairs.

“BIE” refers to the Bureau of Indian Education.

“CO” refers to the BIA or BIE Contracting Officer.

“COR” refers to the Contracting Officer’s Representative.

“Design Handbook” refers to the current edition of the BIA School Facilities Design Handbook.

“DOI” refers to the U.S. Department of the Interior.

“DSRM” refers to the “Division of Safety and Risk Management” which is the Authority Having Jurisdiction and located in Albuquerque, NM.

“Plan Review Procedure” refers to the October 10, 2012, Rev. No. 8, “Safety, Health and Accessibility Design Guidelines for Architects, Engineers and Design Firms”.

“SOW” refers to Statement of Work.

B. CODES AND STANDARDS

The Contractor shall provide a multi-disciplined team to furnish the services required to complete the work. Each responsible team member must be knowledgeable of and provide work in compliance with the most current editions of all applicable codes, standards, design handbooks, plan review procedures, and guidelines as adopted by BIA.

• Value Engineering DOI OMB Circular A-131 Where applicable, the Contractor shall adhere to all federal and state statutes pertaining to archaeological, historical, and environmental clearances which may affect this project. Strict adherence to the National Environmental Protection Act (NEPA) and the National Historic Preservation Act (NHPA), Section 106, will be mandatory.

C. SCOPE OF WORK

1. The complete replacement and repair of a new gym floor system. Remove and dispose of the existing gym flooring and subflooring only to the extent required for new construction.

2. Gym Floor System Installation:

A. Vapor Barrier

1. Install polyethylene with joints lapped a minimum of 6” (150mm) and turned up 4” (100mm) at the walls.

B. Subfloor

1. Install Robbins shock-absorbing pad or approved equal per the manufacturer’s recommendations over 6 mil polyethylene.

2. Place Bio-Channel Star or approved equal subfloor assembly following manufacturer’s guidelines. Stagger end joints in adjacent rows, allowing for a ¼” (6mm) gap between panels. Panels placed on a 45-degree angle to the direction of the maple flooring. Provide 1-½” to 2" (40 to 50mm) expansion void at the perimeter and all vertical obstructions.

3. Install solid blocking at doorways, under bleachers in the stacked position, and below portable goals.

4. Install Bleacher Blocking according to the manufacturer’s recommendations.

5. Place a metal anchor channel in each factory-prepared location in the panel.

6. Anchor each anchor channel in the center pre-routed hole only unless relocation is required due to trimming at a wall or vertical obstruction.

C. Flooring

1. Machine nail maple finish flooring 10” to 12” (150mm to 200mm) O.C. with end joints properly driven up. Ensure proper spacing for humidity conditions. Consult local Robbins Accredited Installation Company “Certified” contractor or approved substituted manufacturer’s equal. Provide 2” (50mm) expansion voids at the perimeter and all vertical obstructions. Ensure even distribution of any expansion rows, with each space not exceeding 1/64” (0.4mm).

D. Finishing A. Sanding

1. Sand per manufacturer’s recommendations.

2. After sanding, buff the entire floor using 100 grit screen or equal grit sandpaper with a heavy-duty buffing machine.

3. Inspect the entire area of the floor to ensure a smooth surface without drum stop marks, gouges, streaks, or shiners.

4. Vacuum and/or tack the floor before the first coat of seal.

B. Finishing

1. Ensure the floor is clean and completely free of dirt and sanding dust.

2. Apply the specified combination of seal, gameline paint, and finish following the manufacturer’s instructions.

3. Buff and vacuum and/or tack between each coat after it dries.

4. Apply game lines accurately after buffing and vacuuming the coated surfaces. Game lines painted between seal coats and finish coats. Layout in accordance with drawings, using current rules of association having jurisdiction. Lines shall be straight with sharp edges in colors selected by the COR.

D. Material Selection:

A. Basis of design: Bio-Channel Star floor system as provided by Robbins Sports Surfaces or approved equivalent.

B. Vapor Barrier

1. 6-mil polyethylene.

C. Replace floor decking with ¾” deck sheeting, glued and screwed to the existing floor joists.

1. 9/16” Zero/G™ shock pad or approved equal

2. Bio-Channel Star Subfloor panels that have been factory prepared to accept anchor channel or approved equal.

3. Metal Anchor Channels.

E. Maple Flooring Manufacturers Association (MFMA) Wood Flooring:

1. 25/32” (20mm) thick x 2-¼” (57mm) width, 2nd&Better grade, Unfinished TGEM, KD Northern Hard Maple, Continuous Strip® XL Flooring as manufactured by Robbins and graded in accordance with MFMA-FJ rules. Flooring will have XLplus™ technology to reduce or eliminate routine spacing for expansion or approved equal.

a. Specie: Northern Hard Maple

b. Seasoning: Kiln Dried

c. Matching: Tongue and groove side-match and end-match.

d. Type: Finger-Jointed (FJ)

e. Pattern: Straight-lay (One directional)

f. Thickness: 25/32”

g. Width: 2 ¼” (57mm)

h. Grade: 2nd and Better

i. Expansion Option: PLUS Feature (Built-in expansion) or approved equal.

j. Factory Finished

k. Robbins EZ-XL or approved equal Factory applied Treatment to prevent side bonding and panelization from water-based finishes. Fasteners

1. Flooring 1 3/4” (44mm) barbed cleats or staples.

2. Channel anchors – 2” Powers SPIKE® anchors or approved equal.

4. Finishing materials

1. MFMA approved sealer

2. MFMA approved finish

5. Gamelines

1. Gameline paint(s) shall be recommended by the finishing materials manufacturer and must be compatible with the finish.

2. Must include lines for up to 3 basketball courts, 3 volleyball courts as well as the center circle logo.

a. School logo shall be applied to center of flooring, aligned with game lines, per owner’s preferred size and colors.

DELIVERY, STORAGE AND HANDLING

A. Delivery of Materials

1. Materials shall not be delivered, stored or installed until all masonry, painting, plastering tilework, marble and terrazzo work is complete, and all overhead mechanical work, lighting, backstops, scoreboards are installed. Room temperature of 55-80 degrees Fahrenheit (13 to 27 degrees Celsius) and relative humidity of 35-50 % are to be maintained. In- Slab Relative Humidity shall be 85% or less using ASTM F 2170 In-Slab Relative Humidity test. Ideal installation/storage conditions are the same as those that will prevail when building is occupied.

2. Materials shall not be stored at the installation location if the In-Slab relative humidity level for the concrete slab is above 85% using ASTM F 2170 In-Slab Relative Humidity test.

6. Construction:

a. Implement proper excavation and foundation preparation.

b. Oversee construction in adherence to design specifications and safety standards.

c. Monitor for quality control throughout the construction process.

7. Drainage Considerations:

a. Incorporate effective drainage solutions to prevent water accumulation under the gym floor.

b. Ensure compliance with relevant industry regulations for drainage.

8. Safety Measures:

a. Implement safety protocols during construction to protect workers and school occupants.

b. Install proper signage and barriers to ensure a secure construction site.

9. Quality Assurance:

A. Manufacturer Qualifications

1. Manufacturer shall be an established firm experienced in products specified in this section and have been in business a minimum of ten (10) years.

2. The Manufacturer shall be a member in good standing of the Maple Flooring Manufacturers Association (MFMA).

3. Materials other than those listed must be approved 10 days prior by written addendum. Materials for substitution will be considered by the COR.

B. Floor Contractor/Installer Qualifications and Certifications

1. The flooring contractor shall be an MFMA Accredited Installer(s) on-site for the duration of the wood floor installation.

2. Flooring contractor shall submit a list of at least three completed projects of similar magnitude and complexity completed under current corporate identity.

C. Floor System Performance Requirements

1. Flooring product meets or exceeds criteria of the following performance criteria:

a. MFMA PUR

b. DIN 18032 Part2 2001

c. DIN 18032 Part2 1991

d. ASTM F2772 Sport Floor Standards

e. FIBA International Standards

f. EN 14904 Standards

2. Independent testing report showing the system passing all criteria shall be provided as part of the bid qualification process and submittal process

3. Surface Appearance to meet one of the below standards.

a. Expansion spaces will not exceed 1/64” (0.4mm) at time of installation and will be spread evenly across the floor with each row of flooring.

b. Expansion spacing will be installed to allow for normal expected increases in Equilibrium Wood Moisture Content (EMC).

4. The wood flooring shall be MFMA-FJ maple.

5. Closed cell foam is not accepted as a resilient pad.

6. Resilient Pad must be laid in a continuous manner.

7. The use of power-actuated or pneumatic anchoring systems is not allowed. The floor system must be anchored using the drilled and pinned method.

8. Metal Channel anchorage shall be provided by 16-gauge (1.5mm) steel sections with subfloor anchor pockets dimensioned to allow a minimum of 1” (25mm) lateral movement.

9. Steel anchor channels shall be of double flange design to capture both side edges of subfloor anchor pocket.

10. Subfloor and metal channel design shall be constructed in a fashion to prevent the over anchorage of the flooring system. Special anchorage tools are not acceptable.

10. Reporting and Documentation:

a. Regularly update project stakeholders on progress.

b. Maintain detailed documentation of the design and construction phases.

c. Prepare a final report upon project completion.

11. Final Inspection

a. Conduct a thorough inspection of the gym floor to confirm alignment with the approved design, as well as adherence to all pertinent regulations, codes, and standards.

The contractor shall arrange for the following as needed:

a. A secure staging area for storing equipment, materials, tools, and supplies, as the contractor shall have sole liability for said items, and

b. Access to and provision of utilities as needed for design and construction.

c. The contractor shall immediately notify the facility manager and contracting officer of any and all issues that may result in a project delay and/or impact work quality or safety.

d. The contractor shall provide all labor, materials, equipment, accessories, etc., required for the project. All materials and equipment furnished for this job shall be in current production and shall be of quality material. Used, shopworn, demonstrator, prototype, reconditioned, or discontinued equipment or materials are not acceptable.

e. All work shall be performed in accordance with current applicable codes. The contractor shall be responsible for obtaining all necessary permits.

f. The contractor shall be responsible for the removal and disposal of parts in accordance with local, state, and federal regulations, and the contractor will provide all the necessary equipment, labor, and materials to safely accomplish said removal and disposal. The contractor shall recover and dispose of the parts per EPA guidelines.

g. All goods and/or services shall comply with applicable OSHA regulations in effect at the time goods are shipped and/or the service is performed. Material Safety Data Sheets

h. Material Safety Data Sheet (MSDS) are required in accordance with applicable regulations. MSDS must be left on site immediately after goods and/or services have been provided.

I. All installations shall be in accordance with the manufacturer’s recommendations.

j. The contractor shall be responsible for removing all construction and packing debris from the work site and keeping a clean, safe work area at all times.

k. If hazardous materials are encountered during execution of the contract, the contractor shall be responsible for removing and disposing said materials in accordance with federal, state, and local statutes and codes. The contractor shall provide BIE with appropriate documentation regarding the disposal of said hazardous materials as requested or required.

m. Safety signs, barricades, and/or other materials will be erected by the contractor to warn visitors and staff away from work areas. The safety of visitors, staff, and contractor’s employees shall be a priority and shall be the responsibility of the contractor.

n. The contractor shall notify BIE in advance of work that may be disruptive to normal operations and parking lot traffic flow. Any shutdown of service and/or utilities must be approved and scheduled with BIE.

o. The contractor shall be responsible for providing written documentation to BIE for any work that may affect BIE’s warranties or existing equipment. Written documentation must be provided and approved prior to the commencement of work.

p. The contractor shall be held liable for the cost of repair or replacement of structures, utility systems and any other parts of the facility damaged by the contractor’s acts of negligence or lack of full adherence to the requirements of the Scope contained herein. The contractor shall be held liable for the cost of repair or replacement of building components and/or vehicle(s) damaged by falling/flying equipment and/or debris.

q. The contractor shall be responsible for the start-up and commissioning of all replaced parts. All systems must be fully functional and operational after installation. If follow-up work is required to correct installation, BIE shall not be charged.

r. Warranties for materials, labor, and equipment will be issued in accordance with industry practices.

s. The contractor shall train key BIE staff in the operation and general maintenance of the perimeter fence and any new equipment. Prior to leaving the job site, the contractor shall provide BIE with all manufacturers' warranty documents upon completion of installation and training.

t. The contractor shall respond to inquiries within twenty-four (24) hours and will service any non-working units on the day of the call during the warranty period. BIE will not be responsible for any additional costs to repair or replace the new gym floor or parts that are still under warranty; the contractor is responsible for all liability.

u. Unless otherwise specified, the contractor shall unconditionally guarantee the labor, and the materials used in performance of this contract within the specified guidelines and recommendations of the manufacturer’s warranty. If any defects or signs of deterioration are noted which in BIE’s opinion are due to faulty workmanship or materials, the contractor shall be notified and shall make the necessary repairs to correct any deficiency in the system at the contractor’s expense.

13. Deliverables:

a. Submit comprehensive Site Assessment Report.

b. Provide detailed gym floor Design Drawings and Specifications.

c. Furnish Material List and Cost Estimate.

d. Deliver a clearly outlined Fence Installation Schedule.

e. Present Inspection Reports.

f. Issue a Project Completion Certificate upon satisfactory completion.

Additional Construction Requirements:

1. Contractor shall provide all labor and materials required to complete the work.

2. Contractor shall provide PPE as required.

3. All damage to Keams Canyon elementary school facilities due to construction or staging operations shall be the Contractor’s responsibility to repair.

4. The Contractor shall provide their own dumpster for removed materials and identify all removed materials that are suitable for recycling. In addition, the contractor shall provide their own temporary restroom facilities and hand washing stations on-site.

5. All waste materials shall be removed and disposed of in a manner in accordance with local, State, and Federal regulations.

6. Construction areas shall be kept in a safe and orderly state during the project period.

7. The Contractor shall make areas safe during demolition and construction phases using safety tools such as cones, snow fencing, and ribbon tape to keep pedestrian and vehicular traffic safe.

8. The Contractor shall provide a copy of the organization’s safety program to the COR after award of contract.

9. At project close-out, the Contractor shall provide three copies of owner’s manuals, “as built” record drawings, installation instructions, blueprint revisions and other pertinent information (including testing reports) on digital CDs and in hard copy.

D. SUBMITTALS:

A. Specification

1. Submit flooring product specification sheet.

B. Shop Drawing

1. Submit flooring product shop drawings as required.

C. Sample

1. Submit one (1) sample of flooring product to COR.

D. Maintenance Guidelines

1. Submit copy of Maintenance Instructions

E. DESIGN/BUILD REQUIREMENTS:

a. Locate, inspect, and determine the capacity of any civil, architectural, structural, mechanical, electrical, and plumbing that will be affected by the construction.

b. The Contractor shall provide A-E services including, but not limited to, construction documents, specifications, and calculations for civil, architectural, structural, mechanical, electrical, and plumbing work as applicable.

c. The Contractor and A-E design team shall meet with the COR and designated Keams Canyon elementary school staff to review design concepts prior to formal submission of construction documents.

d. The Contractor and A-E design team shall locate, inspect, determine suitability, capacity, and all other conditions of the existing site that will be affected by the installation of the new gym floor.

e. Construction documents including specifications are required. The documents shall be prepared by a qualified Architect and /or Engineer(s) of Record, licensed in the State of Arizona for submittal at 70% (preliminary construction documents), 99% (completed construction documents), and 100% (final construction documents), which are to be reviewed with Keams Canyon elementary school and DSRM. Acceptance and /or approval must be obtained prior to moving on to the next stage of design -refer to the Plan Review Procedure and other guidelines available by request.

F. DSRM SUBMITTAL REQUIREMENTS:

1. Three (3) Sets of Full-Size Drawings

2. Three (3) Sets of Specifications

3. Three (3) Sets of All Other Related Documents (i.e., Amendments, Addenda)

4. Three (3) Sets of Full-Size Shop Drawings and Applicable Related Documents

5. DSRM Design Review Submittal Form (Attachment A)

6. DSRM Letter of Transmittal (Attachment B)

7. All documents submitted to the COR for DSRM review shall contain the State of Arizona registered professional seal of the Designer(s) of Record.

8. The Contractor shall provide electronic copies of all documents.

G. Additional Project Requirements:

1. This work shall be performed by a qualified contractor(s) with experience in the design and construction of gym floors.

2. The Contractor shall note any historical status, thus is bound by any requirements relating to construction involving a historical facility.

3. Locate all underground utilities in the construction area(s).

4. The Contractor and A-E design team shall verify that the new construction will not negatively impact the existing site.

5. The Contractor shall engage the services of qualified A-E design professionals with experience in gym floor design, licensed in the State of Arizona.

6. All materials utilized, and work performed shall follow all applicable codes and standards. The installation shall be done in a quality workmanship-like manner.

7. The Contractor shall provide all labor, materials, and appropriate equipment required to complete the work.

8. Testing and monitoring to be performed by a qualified local third-party testing lab.

9. The school may be in use during construction. Schedule any deliveries, utility outages, and building closings with the COR.

10. Comply with all Federal, State, and local environmental erosion control requirements during construction operations. Remove installed erosion barriers, fences, etc. after construction is completed. Submit an erosion control plan to COR.

H. TASK DESCRIPTION

The contractor shall perform the following tasks and deliverables:

1. Mobilization to the Keams Canyon Elementary school site.

2. A secure staging area for storing equipment, materials, tools, and supplies, as the contractor shall have sole liability for said items, and access to and provision of utilities as needed for design and construction.

3. The contractor shall immediately notify the facility manager and contracting officer of any and all issues that may result in a project delay and/or impact work quality or safety.

4. The contractor shall provide all labor, materials, equipment, accessories, etc., required for the project. All materials and equipment furnished for this job shall be in current production and shall be of quality material. 16. The contractor shall be responsible for the start-up and commissioning of all installation. The gym floor must be fully functional and operational after installation. If follow-up work is required to correct installation, BIE shall not be charged.

17. Warranties for materials, labor, and equipment shall be issued in accordance with industry practices.

18. The contractor shall train key BIE staff in the operation and general maintenance of all new equipment. Prior to leaving the job site, the contractor shall provide BIE with all manufacturers' warranty documents upon completion of installation and training.

19. The contractor shall respond to inquiries within twenty-four (24) hours and shall service any non-working units on the day of the call during the warranty period. BIE will not be responsible for any additional costs to repair or replace new equipment or parts that are still under warranty; the contractor is responsible for all liability.

20. Unless otherwise specified, the contractor shall unconditionally guarantee the labor, and the materials used in performance of this contract within the specified guidelines and recommendations of the manufacturer’s warranty. If any defects or signs of deterioration are noted which in BIE’s opinion are due to faulty workmanship or materials, the contractor shall be notified and shall make the necessary repairs to correct any deficiency in the system at the contractor’s expense.

I. DELIVERABLES

1. The Contractor shall review the dimensions, details, markings, samples, product data, and other submittals with the COR and facility manager. Submittals shall contain data regarding components, details, and materials that will be used in the project.

2. A copy of the civil drawings, architectural, mechanical /electrical, and structural drawings prepared by and bearing an Arizona Professional seal shall be provided to the COR for the applicable related tasks.

3. Submit all inspection reports to the COR.

4. The Contractor shall perform its own final inspection of the installation with appropriate checklists and submit this to the COR before the government’s final approval.

5. The Contractor’s inspection checklist report shall include how noted items have been addressed and are to be submitted to the CO prior to securing the government’s final approval.

6. The Contractor shall provide all closed out documents, including but not limited to, warranties, operation, maintenance information, etc. as required by the contract.

J. PERIOD OF PERFORMANCE

The delivery shall be completed within sixty (60) calendar days after the Notice to Proceed, with substantial completion.

K. BACKGROUND INVESTIGATIONS

The Contractor shall be required to submit to a background investigation. The background check process will include, but may not be limited to, filling out hard copy forms, online forms, fingerprinting, and submitting documentation in a timely manner. The contractor shall not be paid for any time associated with this process outside this contract but should include estimated costs in their proposal to meet this requirement. It can take 2-5 weeks for clearance, so it is imperative to start this process as soon as possible.

Typically, the Project Manager/COR and the alternate is to submit paperwork (daily, payroll, testing, and weekly reports). The “Notice to Proceed” will not be issued until the background checks have cleared.
