About this file
This document contains hazardous materials management forms and procedures for transient contractors operating at Patrick Space Force Base (PAFB) and Cape Canaveral Space Force Station (CCAFS) under the 45th Space Wing.
The PAFB/CCAFS Transient Contractor HAZMAT Usage Worksheet is a monthly reporting form that contractors must submit to their Contract/Project Manager by the last business day of each month for the contract duration. The form requires documentation of all hazardous materials used, including Safety Data Sheet (SDS) product trade names, part numbers, manufacturers, container types and sizes, and total quantities used. A separate 45 SW Construction Contractor Spill Notification Form is provided for immediate incident reporting, requiring details on spill location, product type, quantity released, duration, cause, and corrective actions, with instructions to contact 911 or (321) 494-0911 and report to 45 CES/CEIE at (321) 298-7022. The Transient Contractor HAZMAT Tracking Process flowchart outlines the complete lifecycle: contractors submit proposed hazardous materials lists upon contract award, the Installation Hazardous Materials Program (IHMP) Manager reviews and approves materials within five working days, contractors then complete Chemical/Hazardous Material Request/Authorization Forms with current SDSs for each material, the IHMP Manager inputs authorization data into the Enterprise Environmental Safety and Occupational Health Management Information System (EESOH-MIS) within ten working days, and contractors provide quarterly purchase reports by the 10th day of the month following each quarter throughout contract performance, with final reports submitted upon project completion.
