The purpose of this solicitation amendment is to:
1. Incorporate attachment C, Offerors’ Questions and the Government’s Response, in Section J of this solicitation. Please view attachment C.
2. Update Sections B, C, F, and L of this solicitation. The changes/revisions to the Sections are in bold print.
3. Extend Offerors' questions to no later than (NLT) 12:00 noon EST by August 3, 2020. Please view Section L.2.
4. Extend the proposals due date from August 4, 2020 to August 10, 2020, NLT 12:00 noon EST. All other solicitation terms and conditions shall remain the same. End of solicitation amendment.

The Pretrial Services Agency for the District of Columbia (PSA) anticipates awarding Multiple Award Indefinite Delivery Indefinite Quantity (IDIQ) Contracts.

MINIMUM/MAXIMUM ORDER VALUE
THE TOTAL MAXIMUM CONTRACT VALUE, defined as the total sum of payment to the Contractor for all task orders against the IDIQ Contract for the base and options years, shall be between $600,000.00 to $5,000,000.00.
THE MINIMUM GUARANTEE ORDER(S) VALUE is $3,000.00 for each IDIQ Contract.
Section C - Description/Specifications/Work Statement
1.0 INTRODUCTION
The Pretrial Services Agency for the District of Columbia (PSA) is an independent entity within the Court Services and Offender Supervision Agency (CSOSA) in the Executive Branch of the federal Government. PSA has served the District of Columbia for 50 years and is widely recognized as a leader in the pretrial field. PSA's mission is to promote pretrial justice and enhance community safety. Our vision is to thrive as a leader within the justice system through a diverse, inclusive and empowered workforce that embodies integrity, excellence, accountability, and innovation in the delivery of the highest quality services.
PSA promotes community safety and return to Court while honoring the constitutional presumption of innocence. PSA plays a vital role in the District of Columbia's criminal justice system. PSA gathers and presents information about newly arrested defendants and available release options for use by judicial officers in deciding what, if any, conditions are established for released defendants. Additionally, PSA supervises defendants released from custody during the pretrial period by monitoring their compliance with conditions of release and by helping to ensure that the defendants appear for scheduled Court hearings.
PSA supervision gives the defendants the opportunity to participate in a variety of pro-social interventions that decrease the likelihood of future criminal behavior. PSA was created by the National Capital Revitalization and Self-Government Improvement Act of 1997 and was certified as a federal Agency under the Executive Branch in August 2000.
2.0 OBJECTIVE
PSA's strategic goals focus on effectiveness of mission accomplishment, which includes solutions to enhance the supervision of defendants in the community. The provision of treatment services to defendants supports PSA's mission of promoting pretrial justice and enhancing community safety. To assist in meeting this mandate, PSA is seeking a Contractor(s) for the following medical and co-occurring behavioral health treatment services:
(1) Daily Medically Managed Intensive Inpatient Detoxification - American Society of Addiction Medicine (ASAM) Level 4;
(2) Co-occurring Disorder Intensive Outpatient Treatment - ASAM Level 2.1;
(3) Residential Treatment - ASAM Level 3.5 - Clinically Managed High Intensity Services (Adult Criteria);
(4) Transitional Housing - ASAM Level 3.1 - Clinically Managed Low Intensity Residential Treatment; and
(5) Daily Medically Managed Intensive Inpatient Treatment
3.0 SCOPE OF WORK
The Contractor shall provide substance use disorder and treatment services for defendants with single or co-occurring psychiatric and substance use disorders in accordance with all applicable District of Columbia certification standards and applicable accreditation standards. The Contractor shall provide medically managed detoxification, substance use disorders and psychiatric treatment services to defendants who are physically dependent on illicit substances or in need of psychiatric stabilization.
4.0 BACKGROUND
PSA is mandated to provide or facilitate assessments and/or treatment services to defendants ordered to undergo such services as part of the defendants' pretrial supervision. The defendants may have single or co-occurring mental health and substance use issues and may include persons of various backgrounds who may not speak English (i.e. Spanish) or have other physical impairments and/or disabilities.
Detoxification, transitional housing and psychiatric stabilization services are typically provided in residential facilities where care is monitored by 24-hour support staff. Intensive outpatient substance use disorders services are provided in the community and sessions are typically held at maximum three (3) days a week.
5.0 TRAVEL
The Contractor shall transport defendants from his/her point of origin at initial placement. All travel shall be local and within a 50 miles radius of the District of Columbia. All travel is in accordance with the Federal Travel Regulations. Points of origin include the D.C. Superior Court located at 500 Indiana Avenue, NW; PSA's office located at 633 and 601 Indiana Avenue, NW; D.C. jail; any D.C. Department of Behavioral Health (DBH) community mental health or substance used treatment facility;
and CSOSA Re-Entry and Sanctions Center located at 1900 Massachusetts Avenue, SE. The Contractor shall provide round trip transportation to the defendants' court appointments at the D.C. Superior Court or the D.C. District Court located at 333 Constitution Avenue, NW.
The Contractor may facilitate any Court or probation appointments electronically only during the COVID 19 pandemic and only if this is acceptable to the Courts and for Pretrial/Probation matters.
6.0 SPECIFIC TASKS
6.1 Daily Medically Managed Intensive Inpatient Detoxification – ASAM Level 4
6.1.1 The Contractor shall provide psychiatrically enhanced detoxification services to co-occurring diagnosed clients whose physiological and substance use disorder requires medically managed intensive inpatient detoxification on a short-term basis, between five (5) to ten (10) days to stabilize and prepare the defendants for a treatment regimen. Concurrent with their subst ance use disorder, some of the defendants will have prescribed psychotropic medications to treat mental illness.
6.1.2 The Contractor may propose ASAM Level 3.7-Withdrawal Management, Short-term Medically Monitored Intensive Withdrawal Management in lieu of ASAM Level 4, Daily Medically Managed Intensive Inpatient Detoxification.
6.1.3 Defendants shall be referred only by the Government from various PSA programs. The Contractor shall place one (1) to four (4) defendants a week by PSA officials.
6.1.4 The Contractor shall have the ability to support co-occurring psychiatric and substance disorders of defendants, present with other needs and conditions, which includes, but not limited to:
- HIV disease;
- Learning and physical disability;
- Pregnancy;
- Severe alcohol or opiate dependence; or
- Other medical problems.
6.1.5 The Contractor shall administer the PPD Mantoux Tuberculin Skin test to all PSA defendants who report without documented evidence of the test within the previous six (6) months.
6.1.6 If the defendants evaluated are in general medical distress, the Contractor may deny admission until the defendant is medically stable.
6.1.7 The duration of the program will vary in length, between five (5) to ten (10) days for each defendant depending on initial and ongoing assessment, treatment plan progress, program design, and available funding. The length of each stay may be reduced or extended outside of this range depending on the defendants' progress in treatment and other factors.
6.1.8 The Contractor shall not give PSA defendants permission to leave the residential facility without prior coordination and approval of the Pretrial Services Officer (PSO). The defendants are permitted to participate in-group therapeutic outings, such as support group meetings, without prior the PSO's consultation as long as defendants are under the observation of the Contractor's staff at all times. All overnight trips, even if therapeutic in nature, require the PSO's approval.
6.1.9 The Contractor shall provide three (3) nutritionally balanced meals and an evening snack seven (7) days per week.
6.1.10 The Contractor shall ensure that defendants have access to routine and emergency medical care while in -treatment at no additional cost to the Government. The Contractor is not required to fund such services. Contractor shall not bill the Agency for routine medical care performed by the Contractor. The Contractor shall provide reasonable access to necessary medical care to defendants while they are in residence. The Contractor is not required to get PSA's approval before allowing defendants to leave the facility for emergency care, but shall do so for routine care. The Contractor shall provide notification of emergency care to the PSO and the PSO's supervisor by telephone immediately and provide written notification within one (1) business day.
6.1.11 The Contractor shall make referrals or otherwise ensure that defendants are able to access necessary ancillary services.
6.1.12 The Contractor shall ensure that the appropriate numbers of qualified personnel are on-site and accessible at all times, 24-hours per day. A qualified clinical personnel is a licensed staff professional (i.e. nursing, social work, psychologist, physicians, substance use counselors, art therapist, etc.). Qualified clinical personnel shall be on-site and accessible at least 16 hours each day, Monday through Friday and at least eight (8) hours per day on the weekends. If the program does not employ clinical personnel during overnight hours, the appropriate number of support or monitoring personnel shall be on-call and available to respond telephonically and in-person if clinical issues requiring immediate attention arise. All programs shall adhere to the staffing patterns that are required by appropriate state licensing body where the treatment program operates.
6.1.13 The Contractor shall provide services to support defendants with co-morbid conditions and special populations to include the hearing-impaired, visually impaired, mobility-impaired defendants, pregnant women and defendants.
6.1.14 The Contractor shall transport the defendant from his/her point of origin to the Contractor's facility at initial placement.

6.1.15 The Contractor shall provide written discharge summaries to the PSO and Contracting Officer Representative (COR) within 24 hours of discharge from treatment.
6.2 Intensive Outpatient Treatment - (Community Treatment Group Sessions) ASAM Level 2.1
6.2.1 The Contractor shall provide level 2.1 Intensive Outpatient Treatment to substance use disordered and co-occurring diagnosed defendants in accordance with criteria outlined in the ASAM (Third Edition, 2013) and relevant state certification standards.
6.2.2 The defendants shall be referred only by the Government from various PSA programs. The Contractor shall place one
(1) to four (4) defendants a week by PSA officials.
6.2.3 The Contractor shall not refuse to admit defendants into intensive outpatient treatment solely because they test positive for illicit substances at placement if these substances have been clinically determined not to require medical detoxification (e.g., cocaine, phencyclidine, marijuana).
6.2.4 If a defendant is evaluated to be in substance related medical or mental distress (i.e., substance induced psychosis), the Contractor may deny admission until defendant is medically and mentally stable.
Intensive Outpatient
6.2.5 At a minimum, the Contractor shall provide individual therapy sessions every two (2) weeks, individual treatment planning and progress sessions, reporting for sessions at least three (3) times per week for a total of at least nine (9) hours per week of group or individual sessions. The frequency of reporting and number of sessions may decrease in the latter phases of treatment based on the treatment plan and the overall program design. The actual duration of the treatment episode shall be individualized and progress based, but the standard duration shall be at least 12 weeks and not more than 20 weeks. The Contractor may exercise discretion in structuring these sessions on a weekly basis to fit the programs scheduling needs.
6.2.6 The Contractor shall make referrals or otherwise ensure that defendants are able to access necessary ancillary services.
6.2.7 The duration of the program will vary in length for each defendant depending on initial and ongoing assessment, treatment plan progress, program design, and available funding.
6.2.8 The Contractor shall forward attendance information to the Government via email or facsimile each day defendants are scheduled to attend sessions. The attendance report shall include the defendants scheduled to attend, those who attended, and those who failed to attend.
6.2.9 The Contractor shall forward bi-monthly summaries of the aggregate number of sessions attended by each referred defendant to the assigned PSO.
6.2.10 The Contractor shall provide services to support defendants with co-morbid conditions and special populations to include the hearing-impaired, visually impaired, mobility-impaired defendants, pregnant woman and non-English Speaking defendants.
6.3 Residential Treatment – ASAM Level 3.5 - Clinically Managed High Intensity Services (Adult Criteria)
6.3.1 The Contractor shall provide level 3.5 clinically managed high intensity residential treatment to substance using and co-occurring diagnosed defendants in accordance with the criteria outlined in the ASAM (Third Edition, 2013) and relevant states and jurisdictional certification standards.
6.3.2 The Contractor shall provide level 3.2-WM clinically managed residential withdrawal management (social detox) to defendants when necessary. The services shall be provided as a component of level 3.5 residential treatment without additional cost to the Government.
6.3.3 The defendants shall be referred only by the Government from various PSA programs. The Contractor shall place one
(1) to four (4) defendants a week by PSA officials.

6.4 Transitional Housing - ASAM Level 3.1 - Clinically Managed Low Intensity Residential Treatment
6.4.1 The Contractor shall provide level 3.1 Clinically-Managed Low Intensity Residential Treatment (Transitional Housing) as defined in ASAM (Third Edition, 2013) in 30-day increments for up to 60 days.
6.4.2 The defendants will be referred only by the Government from various PSA programs. The Contractor shall place one (1) to four (4) defendants a week by PSA officials.
6.4.3 The duration of the program will vary in length for each defendant depending on initial and ongoing assessment, treatment plan progress, program design, and available funding. In general, the scheduled length of stay will be in 30-day increments and will usually not exceed 60 days of residential treatment. However, the Contractor or Government may request the length of stay be reduced or extended outside of this range, depending on the defendants' progress in treatment and other factors.

6.4.6 Prior to the defendants completing the transitional program, the Contractor shall provide support services to help the defendant secure low-cost permanent housing.
6.4.7 The Contractor's program shall meet all federal, state, and local requirements and the Contractor shall provide, at a minimum:
- Community-based transitional and education services that promote a drug-free lifestyle; individual and group counseling sessions of at least five (5) hours per week.
- Assistance in acquiring stable, permanent employment, including assistance with obtaining GED, employment and computer training.
- Assistance in the reunification of the defendant with his/her children, family, and community.
- Relapse prevention, parenting, and other educational services and training to the defendant and his/her family.
- Services to support defendants with co-morbid conditions and special populations to include the hearing-impaired, visually impaired, mobility-impaired defendants, pregnant woman and non-English speaking defendants.
- Written discharge summaries to the PSO and COR within 24 hours of discharge from treatment.
6.5 Medically Managed Intensive Residential Inpatient Treatment
6.5.1 The Contractor shall provide comprehensive co-occurring enhanced medically managed intensive inpatient treatment services for defendants with both psychiatric and substance use disorders which includes daily therapeutic activities, a protective environment that includes medical stabilization, support and treatment for psychiatric and substance use disorders and daily living support and supervision. Other required services include: physical examination, orders for appropriate tests, and face to face consultation with a physician; psychotropic medication prescribing, dispensing and monitoring, biopsychosocial assessment, evaluation and diagnostic services; co-occurring enhanced individual counseling, group and/or family counseling; recreation and therapeutic activities; case management and comprehensive discharge planning. Programs shall offer integrated treatment for both substance use and mental illness in all program elements. The maximum length of stay is 14 days. Treatment beyond the 14 days shall be approved in advance by the Government. The Contractor shall provide written documentation with justification and an updated Comprehensive Treatment Plan agreed to by the assigned case manager when requesting treatment beyond the maximum length of stay (14 days).

6.5.14 The Contractor shall provide written discharge summaries to the PSO and COR within 48 hours of discharge from treatment.
7.0 TREATMENT FOR THE VISUALLY & HEARING IMPAIRED (OPTIONAL)
The Contractor shall provide professional treatment services to individuals with varying degrees of hearing impairment. When required the Contractor shall employ or contract with personnel skilled in American Sign Language (ASL); have mechanisms for rousing sleeping residents and alerting them to emergencies; and include media (e.g., treatment related DVDs) that include captioning or braille. The services required include ASL interpreters/translators for hearing-impaired defendants while participating in treatment sessions. The location of services will be within the local geographical area of the Washington, DC Metropolitan area. Interpreters will be required to meet defendants at the service location and shall remain with the defendants during the scheduled treatment hours.
ASL services “when or if needed” is anticipated to be required for up to 14 days for Detoxification and Medical Stabilization Treatment Services, up to 180 days for Intensive Out Patient (IOP) Services, and up to 90 days for Residential Treatment and Transitional Housing; however, this is not a guaranteed minimum. Contractors shall invoice only for actual hours of services provided.
8.0 ADDITIONAL REQUIRED SERVICES
8.1 Spanish Language & Culturally Specific Treatment
The predominant limited English proficiency population that PSA serves is Spanish-speaking defendants. The Contractor's language component shall consist of Spanish speaking case managers/therapists, Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) meetings conducted in Spanish, group therapy conducted in Spanish, and case management services conducted in Spanish (i.e., documents, treatment plans, assessments, discharge summaries, etc.).
