About this file

This is a Vendor Reference Form for Johnson County, Texas, required as part of the response to RFP 2026-301 for an Insurance Consultant position. Respondents must provide five (5) references from customers other than Johnson County who can verify the vendor's performance in sales and/or services, delivery, invoicing, and related capabilities. The County prefers government customers as references and requires that references be able to demonstrate the respondent's quality of service and completion of projects of similar size and scope to the work described in the proposal. Each reference entry requires documentation of the government or company name, complete address, contact name and title, business phone and email, contract period, and scope of work completed.

Failure to provide all five required references will result in the respondent being deemed non-responsive and ineligible for award consideration. Johnson County or their designee will conduct all reference checks, and respondent involvement in this process is prohibited; any deviation from this requirement will result in proposal rejection. The form explicitly warns that inaccurate, obsolete, or negative responses from references could lead to rejection of the proposal, establishing reference verification as a critical evaluation criterion in the selection process.
