About this file

This is a specifications document for a biohazard waste collection and disposal contract solicitation issued by Salt Lake County on behalf of the Sheriff's Office, Animal Services, and Health Department. The contract requires the selected contractor to provide collection and disposal services for medical biohazard waste, sharps waste, and animal carcasses through incineration at OSHA-qualified facilities. Services include regularly scheduled pick-ups at multiple locations according to an attached schedule, with the contractor responsible for providing 44-gallon containers, bags, ties, and labels for all divisions. The Sheriff's Office and Health Department require numbered containers for tracking and advance notification of pick-up times, while Animal Services requires weekly collection of animal carcasses and medical waste from freezer units, with the contractor assuming full responsibility for loading and unloading operations. The contractor must maintain all necessary licenses, registrations, and certifications for operations in Utah and Salt Lake County, be permitted as a Waste Hauler through the Salt Lake County Health Department, and provide Certificates of Disposal for each pick-up.

Pricing must be all-inclusive with delivery charges shipped FOB Destination, Freight Pre-Paid, and the contractor shall not charge fuel surcharges, hazmat fees, container fees, overweight charges, or any ancillary fees. Stop charges are waived when visit costs exceed the minimum pick-up fee, and no-waste fees apply only when facilities are closed or waste collection is unsuccessful. Pricing evaluation is based on the bidder's grand total from the pricing schedule. Bidders must demonstrate three years of successful experience providing related services and supply three references. The contractor must provide unlimited access to training materials on proper handling and container use at no charge and be prepared to respond to emergency on-call requests within 24 hours. Containers may exceed 75 pounds, and overweight fees are not permitted.
