About this file

This is a Small Business Enterprise (SBE) Subcontractor Intent Form (BEH-FRM-610.SAW) used by the City of Dallas Business Enterprise Hub (BEH) in conjunction with the Lift Stations and Diversion Structures Improvements project. The form documents the prime vendor's intent to subcontract with a certified SBE for engineering services related to investigation, study, preliminary design, design development, construction plans and specifications, bid phase services, construction administration, and start-up activities for lift station and diversion structure modifications, repairs, and replacements. The form requires identification of the project name, solicitation number, SBE vendor name, prime vendor name, specific goods or services to be provided, applicable NAICS codes, and the SBE's current certification status and expiration date.

The form establishes SBE participation requirements by documenting the prime vendor's total contract amount, the contract-specific SBE goal percentage as stated in the solicitation, the SBE vendor's participation amount, and the SBE vendor's percentage of total contract value. Both the prime vendor and SBE subcontractor must certify that the SBE will perform work within its certified NAICS code under a Commercially Useful Function (CUF) using its own workforce and supervision. The form requires prior BEH approval for any reduction, substitution, or reallocation of the subcontractor's scope, value, or percentage participation. SBE certification must be maintained current and valid throughout the contract duration, and certification may be obtained through the DFW Business Council, North Central Texas Regional Certification Agency, Women's Business Council Southwest, or other approved agencies. Both parties must sign the form, with signatures from authorized officers of both the prime vendor and SBE subcontractor required for submission to the City of Dallas.
