About this file

This is a DRIVE Policy Engagement Score and Past Commitment Form used by the City of Dallas to evaluate vendor qualifications for state and local contract opportunities, including the Lift Stations and Diversion Structures Improvements project in Texas. The form requires respondents to document their historical performance in meeting certified subcontracting commitments by providing information on three relevant projects completed within the last five years, including project names, owners, contract values, subcontracting program types, initial commitments, and final utilization percentages. Respondents must also provide owner points of contact with names, email addresses, and phone numbers for verification purposes.

The evaluation framework awards points across four categories: three points for past subcontractor utilization, one point for formal Mentor-Protege program participation, one point for active engagement with Dallas-based industry organizations supporting small and local business development, and one point for timely payments to subcontractors on recent projects. To earn the timely payment point, respondents must submit at least two types of supporting documentation such as system-generated payment reports, subcontractor letters confirming receipt, compliance audit reports, or internal payment logs. The form requires certification by a company officer attesting to the accuracy and completeness of submitted information, with acknowledgment that false or misleading information may result in rejection of the response or other actions under City policy.
