About this file
This is a proposal evaluation score sheet for the Utah Department of Transportation (UDOT) Procurement Services Request for Proposals for Regional Welding and Fabrication Services (Solicitation #UDOT270026MP). The RFP seeks qualified firms to provide welding and fabrication services under UDOT regional contracts, with anticipation of multiple contract awards. Prospective contractors must comply with Utah Department of Transportation standards and specifications. The evaluation process is structured in three stages: Stage 1 requires mandatory minimum requirements including current business license, five years of experience, and no claims of nonperformance; Stage 2 involves detailed technical evaluation across four criteria totaling 500 possible points; and Stage 3 evaluates cost proposals. Proposals must achieve a minimum score of 300 points (60 percent) in the technical evaluation stage to proceed to cost evaluation.
The technical evaluation criteria include Experience and Services (300 points, weighted at 60 percent), Qualification and Expertise of Personnel (100 points, weighted at 20 percent), Process for Scheduling Services (75 points, weighted at 15 percent), and References requiring a minimum of two (25 points, weighted at 5 percent). The cost evaluation component is worth 100 total possible points, with the lowest-cost offeror receiving maximum cost points and all other offerors receiving points calculated using a ratio formula based on their proposed cost relative to the lowest bid. Offerors proposing costs more than double the lowest proposed cost receive no points. The total evaluation comprises 600 possible points across all three stages, with scoring criteria ranging from Poor (1) to Excellent (5) based on how proposals address RFP requirements.

Evaluation of Cost Proposals: The offeror with the lowest total cost will receive the maximum available Cost points. All other offerors will receive points as determined by the ratio* of their cost to the lowest offered cost. Final cost scores will be calculated based on the following:
*Ratio Calculation: Points assigned to each offeror’s Cost Proposal will be based on the Lowest Proposed Cost. The offeror with the Lowest Proposed Cost will receive 100% of the cost points. All other offerors will receive a portion of the total cost points based on what percentage higher their proposed cost is than the Lowest Proposed Cost. An Offeror whose proposed cost is more than double (200%) the Lowest Proposed Cost will receive no points. The formula to compute the points is: Cost Points x (2- (Proposed Cost/ Lowest Proposed Cost)).
