APPENDIX A: PAST PERFORMANCE QUESTIONNAIRE

Complete one set of letters and forms for each Past Performance reference in which performance occurred within the three years of initial solicitation release. Additional space or blank sheets may be added to answer any questions.

PAST PERFORMANCE – COVER LETTER

Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question as defined below.

PERFORMANCE RATING

EXCEPTIONAL (E): Performance meets contractual requirements and exceeds many to the customer’s benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

VERY GOOD (VG): Performance meets contractual requirements and exceeds some to the customer’s benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

SATISFACTORY (S): Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

MARGINAL (M): Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.

UNSATISFACTORY (U): Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
