SectionTopicQuestionAnswer
N/AGeneral QuestionThe response is 4PM 20 November 2017, is that Pacific Standard Time? That is correct.
RFP AttachmentsProperty ListThe J3 is 1 page, is that accurate or are there pages missing? For instance SOW 4.3 relates to 43 pieces of IH equipment that need calibration……….. where are those on the list? The property in J3 will be supplemented with an additional list. Once the list is posted it will answer your question.
RFPAdditionally, in the Q&A (SOW 4.3) you answered that currently there are 3 service providers that come on the calibrate large medical equipment………. Does the government pay for this? Does the contractor? The Prime Contractor is responsible for calibrating the equipment, either as a prime, or through a subcontractor.
It states that the Ames Metrology Lab can be used for calibration of equipment, is the government paying for that as they do at MSFC? Or is the contractor responsible? We could not find this reference in the documentation. The Metrology Lab at Ames was closed in 2004. The contractor is responsible for the calibration of equipment.
SOWIn historical data it does not mention X-rays, how many are performed annually? We understand that they are referred to a local vendor………..At whose expense?X-ray services are not provided at Ames. Occasionally, examination of an occupational injury or illness will require an x-ray. Where needed, x-rays are provided through a local service provider, and paid for by the HU contractor. These are infrequent and we estimate no more than 5 per year would be required.
SOWIn Q&A’s you don’t answer who pays for the Allergy shots (SOW 3.7)……. Is it the government? The patient? The contractor? In most cases allergy shots are for non-occupational cases and are provided through the patient's private provider. For an occupational case, where the work environment was the cause of the allergy, allergy shots would be provided and paid for by the HU contractor.
RFPPast Performance # of Total SubmissionsThe Offeror and each Major Subcontractor, if any, (reference provision L.12, Major Subcontractors) shall identify a maximum of three (3) relevant contracts held within the past three years.. Please clarify whether this is a maximum of 3 contracts for the entire Team (Prime and any Major Subcontractors) or a maximum of 3 contracts each submitted by the Prime, and a maximum of 3 contracts for each Major Subcontractor?3 for the Offeror (Prime) and 3 for each Subcontractor.
L.12Major SubcontractorsPlease clarify whether the definition of a Major Subcontractor means that the Major Subcontractor must meet just one of the two definitions as noted in paragraphs 1) and 2) of L.12? We ask becuase if a Major Subcontractor did Mission Assurance, they would clearly meet the definition for paragraph 2) but we would not be able to demonstrate them meeting paragraph 1) since all the Mission Assurance work is allocated to IDIQ task orders and there is no guaranteed level of effort there, just the $38M ceiling value.
If you mean that a Major Subcontractor needs to meet both 1) and 2), then will the government permit contractors to assume, just for the purposes of defining a Major Subcontractor, the the $38M ceiling for the IDIQ SOW be used as part of the calculation of performing $2M annual revenue? The Major sub only needs to meet one of the requirements of the section.
L.16.1Past Performance Proposal FormatQuestions 1-10 will take nearly a page just to answer, even though each answer will be one word or a short phrase, if we are unable to put them into a table and use 10 point font single spaced. We request that an exception be made to the font/spacing restrictions in this case to allow contractors to focus the limited page count on the narrative responses to questions 11-13.Yes, 10 pt. font single spaced will be allowed in tables.
L.16.1Past PerformanceThere is page allocation for each past performance project; however we do not see any page allocation for an introduction to Factor 3. Please clarify. We do not require an introduction for Factor 3. The information regarding past projects is sufficient.
L.11Submission FormatPlease confirm that there are no hard copy submissions required. Electronic submission only? Submis via FBO?No hard copies will be accepted. Electronic submission through FBO is required.
L.16.1PPQ Submission DatePlease clarify the due date for the PPQs to the contract specialist. Instructions in paragraph b.) of this section note the due date is noted in Block 20C of the SF-33; however there is no 20C. Do you mean block 9 (4:00 pm PDT on 20 November 2017)?Correct. This will be revised.
B.1Section B.1 vs. SOWIn Section B.1, the references to the SOW sections do not match the numbering scheme in the SOW itself. For example, Section B.1 CLIN 004 Emergency Response, refers to SOW Section 3.3. However, SOW Section 3.3 is Ergonomics Program not Emergency Response. This inconsistency appears in several of the CLIN/SOW paragraph references. Please clarify whether B.1 or the SOW will be changed.B1 will be revised
B.1Section B.1References to SOW Section 3.8 are duplicated on multiple CLINs in the options years and SOW Section 3.10 references are missing.B1 will be revised
B.1Section B.1CLIN 023 should say "IDIQ" rather than "Core."B1 will be revised
B.1Section B.1The following CLINs refer to the wrong Option Year: 043, 044, 054, and 055.B1 will be revised
L.13.bTables Format ClarificationIn the Question and Answer (page 11) the government responded saying "We will revise page limits when the final RPF is released. Tables, figures and Graphics will be allowed a 10 pt. font." However the Final RFP notes that Graphics and Charts may contain 10 pt font. There is no guidance in the final RFP regarding the font size for tables. Please advise. That was an oversight. The RFP will be amended to address this concern.
N/AGeneral QuestionWill there be an Industry Day scheduled? An industry day will not be held
N/AGeneral QuestionWhat is the incumbent’s name/contract number? The incumbent is Earth Resources Technology, Inc. The current contract number is NNA13AA12B.
SOWIn order to determine the replacement schedule for the AED's the Prime must know the purchase date of those AED’s on-site.The contractor is responsible for creating a maintenance SOP in accordance with NPR 1800.1 and APD 8715.1. However, due to the lack of available information, we will move the ordering and maintenance requirement to the IDIQ section of the SOW at 4.1.5.
SOW 3.7.1Section 3.7.1 requires the Contractor to make all files available electronically. Please provide the number of paper medical records to be converted to electronic form and inserted into Medgate under this Contract. For the past few years, only electronic records have been created. There are still about 5 to 10K paper charts/folders from prior years. These should be converted within 5 years.
GeneralIs there a separate building for the non-medical staff, e.g. Industrial Hygienists, occupational safety, etc?Medical staff are located in building N215. Non-medical industrial hygiene and occupational safety staff are primarily located in building N237.
SOWWhat are the x-ray quantities per year? Are the services to be provided by the Offeror?X-ray services are not provided at Ames. Occasionally, examination of an occupational injury or illness will require an x-ray. Where needed, x-rays are provided through a local service provider, and paid for by the HU contractor. These are infrequent and we estimate no more than 5 per year would be required.
RFP AttachmentsIs there gym furnished equipment? If so, please add to GFE attachment. An equipment list will be provided.
RFPWill NASA provide information on the current vendors for x-rays, laboratory services, medical equipment calibration, IH equipment calibration, AED manufacturer.Any local service provider for X-Ray or lab services is acceptable. An example of one provider of local laboratory services currently used is Quest Diagnostics. All equipment calibration is controlled by APR 8730.1, Metrology and Calibration. Examples of recent service providers include: SIMCO Electronics, CMM Systems, Instron Calibration Services, Precision Measurement, Inc., and Rice Lake Weighing Systems. The AED manufacturer is Philips.
RFPWill there be a page count relief for cover page, TOC, etc?RFP will be updated to state that the cover page and several other items will not be included in the page count.
RFPDo the header and footer need to be double spaced with the rest of the document? No.
SOW 2.0Scope of Work Section 2.0 states: Some equipment, materials, and supplies will be supplied by the Government, while others are to be supplied by the contractor as described in Section 2.4. Section 2.3 and 2.4 are missing from the SOW document. Please provide section 2.4.The SOW will be updated to address this.
B.1Section B.1 vs. SOWThe numbering convention of Section B does not align with the J-1 Scope of Work references. Will Section B be updated?Yes it will be updated to match the SOW.
N/AGeneral QuestionAlso, does NASA anticipate that potential Offerors for the subject procurement will require additional time to prepare the Cost Volume based upon NASA’s answers to questions? If yes, will an extension to the Due Date be issued?Any form of extension will be dependent on the quantity and contractual significance of questions asked as well as the perceived effort it will take for Offerors to make adjustments to their proposal.
SOWWhat is the inventory of the ARC Health Unit Crash Cart and the historic cost to keep it replenished? An updated inventory list will be provided in the supplemental data attachment J6. The inventory is for the entire health unit, including crash cart, ambulance, and disaster trailer. Historical purchases for all inventory, eyeglasses, and vaccinations from 2016-2018 are approximately $100,000 per year.
SOWWhat is the inventory of the ARC Health Unit Ambulance and the historic cost to keep it replenished? An updated inventory list will be provided in the supplemental data attachment J6. The inventory is for the entire health unit, including crash cart, ambulance, and disaster trailer. Historical purchases for all inventory, eyeglasses, and vaccinations from 2016-2018 are approximately $100,000 per year.
SOWIs the contractor responsible for the vehicular maintenance of the Ambulance? If so, what is the make, model, maintenance history, and historic annual cost of maintenance? No. The contractor is not responsible for the maintenance of the ambulance vehicle (Wheels, oil, brakes, etc.)
SOWWhat is the inventory of the ARC Health Unit Disaster Response trailer and the historic cost to keep it replenished? An updated inventory list will be provided in the supplemental data attachment J6. The inventory is for the entire health unit, including crash cart, ambulance, and disaster trailer. Historical purchases for all inventory, eyeglasses, and vaccinations from 2016-2018 are approximately $100,000 per year.
SOWIs the contractor responsible for the maintenance of the Disaster response Trailer? If so, what is the make, model, maintenance history, and historic annual cost of maintenance? No. The contractor is not responsible for the maintenance of the disaster response trailer vehicle (Wheels, roof, brakes, etc.)
SOWWhat obligations does the contractor have should the ambulance be out of service for any reason? Will the contractor be responsible in any way if the Ambulance is out of service? If so, what obligations / responsibilities does the contractor have if the Ambulance is out of service?Ocassionallly the ambulance will be out of service. When it is out of service the contractor will need to communicate the matter to NASA Dispatch and make alternate plans to respond to emergencies. Alternate responses are normally conducted using another government non-ambulance vehicle.
SOWWhat are the historic quantities of lab tests in order to determine overall pricing. There are many types of tests that are conducted. Some at the Health Unit and some off-site by laboratories. For the off-site labs work there may be approximately 500-700 per year. But that can vary depending on how many people need exams.
SOWWhat are the types of x-ray services offered? Are the costs of the x-ray read paid for by Government or Contractor?X-ray services are not provided at Ames. Occasionally, examination of an occupational injury or illness will require an x-ray. Where needed, x-rays are provided through a local service provider, and paid for by the HU contractor. These are infrequent and we estimate no more than 5 per year would be required.
SOWOne more question, will ARC please provide the estimated number of exams and exam type? The exams vary in type and quantity. The types of exams that will need to be provided is shown in NPR 1800.1 D. The total number of medical surveillence exams for recent Fiscal Years is included in figures 9 and 10 of Attachment J6. No further information will be provided.
Will ARC please confirm if Contractor personnel identified in the Supplemental Data will be offered Clearance Exams. If so, what is the estimated number of annual exams?Typically, each contractor is responsible for Clearance Exams for their personnel. In unusual cases, for example a contractor participating in a research study, they might be given a clearance exam at the health unit.
J.8, J.11IDIQ Labor categoriesSome labor category descriptions are missing. The SLCs and additional labor categories provided in attachment J.11 don't seem to have the same labor categories that the bidders can use to map into the J.8. For example, J.8 is requiring Administrative Support category to priced, however, there is no level provided, so there doesn't seem to be a way to map this Administrative Support category into any of the Administrative Labor categories provided in J.11. Will the government provide all the labor category descriptions to ensure that realistic and consistent DL rates are proposed, or provide a way to map the labor categories in the J.8 template to the J.11 template?J8 and J11 attachments will be updated.
Attachment J8Cost TemplatesCan offerors adjust the formulae/format to ensure consitent and correct calculations are provided? For example, there is a #REF error in cell K43 in Attach J8. Will the govt. provide an updated file to correc the error or are contractors allowed to update the file?J8 attachment will be updated.
Attachment J11 & J8IDIQ Labor categoriesSeveral of the labor categories listed in Section J.8 for IDIQ support (SOW Section 4) appear to be directly related to Medical Support Services (Physician, Nurse, Medical Assistant, Fitness Center Monitor, Optometrist). However, in reviewing SOW Section 4, there appears to be no scope area where such services performed by those labor categories is permitted. Please clarify what area of Section 4 allows for IDIQ performance of Medical or Fitness Center support services or revise Section J.8.The Physician, Nurse, Medical Assistant, Fitness Center Monitor, and Optometrist are all included in the CORE. Please review. Please see SOW Section 4.1 cross functional support. In the case of a surge in work due to unforeseen circumstances, additional staffing could be required for a short time in any performance area. These hours are for proposal purposes only to set a maximum contract value.
Attachment J11CORE Labor CategoriesSeveral of the labor categories listed in Section J.11 SLCs for CORE support (SOW Section 3) appear to be directly related to System Safety and Mission Assurance (Physician, Nurse, Medical Assistant; however, Fitness Center Monitor, Optometrist). However, in reviewing SOW Section 3, there appears to be no scope area where such services performed by those labor categories is permitted. Please clarify the purpose of bidding these labor categories in the CORE, if all of the work performed by a person in a given labor category would be performed as an IDIQ task? The Physician, Nurse, Medical Assistant, Fitness Center Monitor, and Optometrist are all included in the CORE. Please review. Please see SOW Section 4.1 cross functional support. In the case of a surge in work due to unforeseen circumstances, additional staffing could be required for a short time in any performance area. These hours are for proposal purposes only to set a maximum contract value.
L.14.1-1ARC CampusIn the sentence "The ARC campus consists of NASA organizations, contractors, commercial tenants, university personnel, and students with varying levels of knowledge of NASA SMA requirements and budget constraints." does budget constraints apply to NASAorganizations also or only the other entities listed after NASA (e.g. contractors, tennants,... )?The budget constraints referenced are for NASA only. The question will be revised for clarity.
L.14.1- 11Risk ManagementIn the sentence "ARC project teams must find design solutions for various types of problems from building a new facility to designing an instrument or spacecraft with different risk profiles…", does the preposistional phrase "different risk profiles" apply to all three examples or only the spacecraft?Applies to only to the spacecraft. This question will be revised for clarity.
L.14.1-11Risk ManagementCan the government provide an example of a risk profile referenced here? There are varying interpretations.Higher risk tolerance, lower cost. A CubeSat would be an example. Unmanned, low cost, short life. Higher risk of mission failure accepted for a lower cost and shorter schedule.
3.1Occ SafetySOW states"Conduct annual reviews of safety programs to meet......" and "NASA also requires annual reviews of five additional occupational safety programs each year, so that each safety program is reviewed at least once every three years". It is not clear how the plurals of safety programs here is meant to apply to this contract. Clarify which of these requirements apply to ARC only and if this implies ARC has to support reviews at other centers. All reviews of Safety Programs are for ARC specific programs, not any external programs.
4.6.2Risk ManagementThe sentence " Prepare project RM plan in accordance…" appears to be an outlier compared to similar responsibilities listed under SS, QA, and SWA whereby the contractor can "prepare or review", or "perform or review"...plans. This section makes no mention of the contractor reviewing said plans, only preparing them. Is reviewing them not desired?Yes, reviewing them also. Additional detail of duties would be provided at the task order level. The IDIQ sections provide for a general overview of the types of work the contractor can expect to perform.
4.6.5SWANo mention of IV&V here. Mission or Safety Critical software as determined by the processes in NPR 7150.2 is reviewed by IV&V. This ususally involves the support contractor providing artifacts ,other materials, and communication with the IV&V facility to aid in their review. There is no mention of the contractor providing any support or assistance with this interface. Is this not desired?NASA may need IV&V support. Any specific details would be included in a task order.
RFPDelivery MethodThe SF33 says '0 copies'; with the proposal being delivered to the fbo.gov web site. The "Questions and Answers from Industry 1" document, last item on page 14, states a delivery requirement of 3 electronic copies. Section L.11.b states: "The Offeror shall submit one electronic copy of the entire proposal to https://www.fbo.gov. The Offeror shall deliver proposals to the Contract Specialist/Contracting Officer and shall not leave proposals unattended..." while Section L.11.d states: "The Offeror shall submit the electronic proposal through the Federal Business Opportunities website...." Please confirm that no CDs are required for this submission and that the only submission is 1 copy to be submitted via the fbo.gov web site. One copy is the correct quantity. The coversheet will be revised. No Hardcopies, CD's, Thumb Drives, memory sticks, or E-mailed proposals will be accepted.
RFPVolume II, Business ProposalWould the offeror be permitted to submit the Cover Letter, Contract Offer, OCI Mitigation Plan, and Government Property Management Information in PDF format? If not, and these must be submitted in Microsoft Word, would the government please provide the contract offer portion of the solicitation in Microsoft Word format? The RFP is being revised to allow PDF submittals
RFPFormatMay graphics and figures be single-spaced?Yes.
RFPIf subcontractors need to submit confidential employee compensation plans, how would the government like to receive these documents?Subcontractors should submit via e-mail. The name of the prime contractor, and a reference to the proposal number should be included with their submission for tracking purposes.
Attachment J7Should the Period of Performance for the FFP Core proposal sheets for contract years 2 through 5 be changed from 10 months to 12 months?Yes this revision will be made.
Attachment J8On the FFP IDIQ Worksheet should the Nurse be a RN or a LVN?The pricing sheet will be revised. Depending on the circumstance this could be an RN or LVN.
RFPThe Statement of NASA/OPM Job Series does not include a Certified Health Physicist. What is the annual salary for the Certified Health Physicist? It is the same as the senior health physicist as the senior health physicist is expected to also be certified.
GeneralHas the list of interested bidders been released? No, due to the lapse in time between the Draft and the Final RFP we are not releasing any formal list. However, you may mark yourself as an interested vendor on the FBO website under the solicitation.
RFPParagraph L.8 makes Government property available to the Contractor in accordance with FAR 52.245-1. That regulation in (h) (1) states that the Contractor shall not be liable for loss of Government Property unless one of three conditions apply. One of the conditions is if the property is insured by the Contractor. Does the Government want the Contractor to procure property insurance and be reimbursed for the insurance premium as a direct charge to the Contract? No, the contractor will not be required to purchase property insurance.
SOW 3.4What Medical team member or members handle the 24/7/365 support coverage? This is supported by industrial hygiene and / or safety personnel. The medical staff has no direct ties with this requirement.
SOW 3.7In order to determine the cost for training and training materials how many members are on the MERT team?Historically the MERT team has varied from 100 in the past to the current 10-20, excluding the Health Unit staff, which is also considered part of the MERT team.
SOW 3.7Can you please provide the current staffing matrix for the Health Unit? The RFP states a WYE of 5.1. Is the Mid-Level Provider (Nurse Practitioner or Physician Assistant) full or part time? Does the matrix include a RN, LPN, or other medical professionals? The current WYE is approximately 5 WYE. We will not provide the staffing mix of the current contract as a specific structure is not called out in the SOW.
SOW 3.7Can you please provide what type of medical surveillance exams are performed historically (lead, asbestos, etc.) and what the number of each type of exam is performed per year. The exams vary in type and quantity. The types of exams that will need to be provided is shown in NPR 1800.1 D. The total number of medical surveillence exams for recent Fiscal Years is included in figures 9 and 10 of Attachment J6. No further information will be provided.
SOW 3.7The SOW requires the contractor to stock the clinic with consumable and non-consumable supplies. Can you please provide the inventory list and the annual spend for the clinic suppliesAn updated inventory list for all GFP will be provided. The inventory is overall for the health unit, including crash cart, ambulance, and disaster trailer. Historical purchases for 2016-2018 are approximately $100,000 per year.
SOW 3.7The SOW requires the contractor restocking the MERT trailer. Can you please provide the inventory list and the annual spend to restock the trailer. The "MERT Trailer" is the disaster trailer referred to in the SOW.
SOW 3.7The SOW requires the contractor restocking the ambulance. Can you please provide the inventory list and the annual spend to maintain the ambulance supplies.
SOW 3.7The SOW requires the contractor restocking the crash cart. Can you please provide the inventory list and the annual spend to maintain the crash cart.An updated inventory list will be provided in the supplemental data attachment J6. The inventory is for the entire health unit, including crash cart, ambulance, and disaster trailer. Historical purchases for all inventory, eyeglasses, and vaccinations from 2016-2018 are approximately $100,000 per year.
SOW 3.7Attachment J6 “Medications dispensed by the Ames Health Unit” – can you please clarify if the numbers provided are the number of tablets dispensed or the number of bottles of the medication dispensed. It is a full container or bottle dispensed.
SOW 3.7Can you please provide the contents of a “travel pack”Travel packs vary in content to meet the needs of the traveler depending on the location on the planet were the person is traveling.
SOW 3.7Can you please provide how the billing is handled for x-rays taken at an outside facility?X-ray services are not provided at Ames. Occasionally, examination of an occupational injury or illness will require an x-ray. Where needed, x-rays are provided through a local service provider, and paid for by the HU contractor. These are infrequent and we estimate no more than 5 per year would be required.
SOW 3.8Can you please provide the current staffing matrix for the Fitness Center? The RFP states a WYE of 1.0 though it requires both a Fitness center manager and fitness center monitor. Additionally, the "NASA Ames Fitness Center Class Schedule" provided in the RFP lists four instructors.Currently, there are 2 WYE on staff and two contract trainers. Attachment 10 is being revised.
Section L.13(b)RFP requirement stating, “… all pages of the proposal including all attachments shall be double-spaced…”Please confirm that this requirement does not include tables. The questions in this table have been double space and give you an idea of the resultant effect. The RFP is being revised to allow 10pt. Font and single spacing.
L.15.3.a.2.b (page 78)This requirement states that “The Offeror shall provide proposed unburdened prime and subcontractor direct hourly labor rates for each labor category listed in the IDIQPS.” The responses to Q&A from industry day and Column I from Attachment J8 in the final RFP labeled “Maximum Sub Labor Rate (Burdened)” indicate that the Offeror shall provide proposed unburdened prime direct hourly labor rates and fully burdened subcontractor hourly labor rates for each labor category listed in the IDIQPS.”Would the Government please confirm that the Offeror shall provide unburdened prime direct hourly labor rates and fully burdened “major subcontractor” hourly labor rates for each labor category listed in the IDIQPS?Yes, that is correct. The RFP will be revised to reflect that only fully burdened rates are required for subcontractors.
L.15.3.a.2.d (page 79)This requirement states that “The IDIQPS includes 52,720 labor hours per year distributed between the prime and subcontractors as listed on the IDIQPS.” However, Attachment J8 includes a total of 53,978.Would the Government please confirm the correct number of hours?52,720 is the correct number of hours for this effort. The IDIQ pricing sheets will be revised accordingly.
B.5 Total Contract ValueThe Quantity (number of months) included in the Option 1-4 tables (pages 12-14) in the RFP is 12, however, the Quantity (number of months) included in the FFPPS Years 2-4 tabs of Attachment J7 is 10.Would the Government please confirm that the Quantity in Attachment J7 should be 12 and not 10?Yes, this is correct. The pricing sheets will be updated to reflection options 1-4 at 12 months instead of 10.
L.11.bL.11.b states “The Offeror shall submit one electronic copy of the entire proposal to https://www.fbo.gov. The Offeror shall deliver proposals to the Contract Specialist/Contracting Officer and shall not leave proposals unattended. The Offeror shall be aware that heightened and varying security requirements may preclude or delay access to the Center; however, such circumstances will not provide a basis for acceptance of a proposal that arrives at the place specified after the exact time and date specified.” This paragraph implies that the Government requires submittal of hard copies.Would the Government please confirm that hard copies are not required and that Offerors must submit only one electronic copy to FBO.gov?The language for this is being revised. You are correct that only one electronic copy is required and must be submitted through the FBO.gov website.
Q&AIn the Q&A, the Government states that there are currently two (2) FTE staff at the FC.Is there any coverage of the FC when personnel are conducting a class in the AM or PM?There currently are 2 FTE employees. The Historical Data Attachment (J10) will be revised to state 2 as well. The typical process is that both employees should be available during the time the classes are being taught.
Q&AEmergency ResponseCan the requirement of a physician riding along with the ambulance be adjusted to include a different, qualified provider? The physician only rides in the NASA Ames ambulance and is required to do so. Any injury requiring transportation to a local hospital is taken over by the county ambulance. NASA does not require a physician to ride off-site with the injured person.
Q&AIn the Q&A the Government states that regarding the AEDS the current plan is for the Contractor to purchase and install 30 new AEDS.Can the Government clarify if the Contract must accommodate the 30 AEDs under the firm fixed price? Or will the Government consider this a cost-plus component? This particular requirement is being moved to the IDIQ section to allow for more flexibility when units must be purchased.
Q&AVehicles provided by fleet servicesIs the ambulance also provided and maintained by fleet services? The ambulance vehicle is provided and maintained by the government.
Attachment J6 Supplemental Data List1500 contractor employeesWhat is the historical frequency that contractors use health unit and fitness center and for what services?Contractor employees do not use the health unit for routine care. For acute injuries a contractor may get medical attention. For 2016 there were 32 occupational injuries, and the first half of 2017 there were 12 injuries. It is estimated that half were contractor. The Fitness Center is open to all NASA badged employees and is used by both Civil Servants and NASA contractors.
Attachment J6 Supplemental Data List1,200 civil service employees who are eligible for health maintenance medical exams (55% receive exams)What is the year over year change to this percentage and the total eligible population? Request a minimum of 3 years of data. Is there any expectation for an increase/decrease to the rate of use?The percentage of Civil Service employees taking advantage of Health Unit exams has typically ranged between 40 and 60 percent. Yet the exams are available to all NASA Civil Servants and the percentage could be as high as 100% .
SOW 3.8, Fitness CenterIn the SOW the Government states that “Classes shall be provided by the fitness center manager or fitness center staff.”Is there sufficient space within the fitness center to hold these classes or are these classes held at a different facility on base? There is sufficient space within the fitness center to hold the classes.
Attachment J6 Supplemental Data ListApproximately 30 laser users. The laser inventory includes 150 lasers of all classes, including approximately 29 Class 3B and 50 Class 4 lasers.Do all laser users handle class 3B and 4 lasers? If not, can the government provide any metrics on how many users access each class?Laser users means – researchers who directly use lasers

There are approximately 25 laser permits issued each year There is really no distinction on controls or coverage required for Class 3B or 4 lasers experiments There is one high energy Class 4 laser experiment that requires additional coverage but that is accounted for in the 25 laser projects estimate There are about 4-5 lower class laser projects using range finding outdoor lasers that are also evaluated and are covered under the 25 laser project estimate above

Attachment J6 Supplemental Data ListAbout 1200 flu shots are administered annually. What is % change year over year? Is there any expectation to add eligible civil servants? Or does NASA keep this volume consistent year over year? The volume has been fairly consistent but could change if the NASA civil servant population changes significantly. It's first come first serve until the supply is gone.

SOW 4.54.5 Employee Assistance Program (EAP)EAP services can be priced based on the size of the population and number of sessions allowed per employee/issue. EAP is usually not priced based on a per counselor rate, however we do not see the requirement for EAP under 3.7 Medical Services, or anywhere in the CORE.Historically this has been at a per/employee rate. However this is an IDIQ requirement and the specific data of who is covered under EAP would be provided at that time, which would allow the contractor to price the task order accordingly.
