NAME OF CONTRACTOR REQUESTING THE REFERENCE:
The purpose of this questionnaire is to obtain information regarding the quality of the above-named Contractor’s past performance relative to a contract, completed or in progress, at your company/agency. NASA Armstrong Flight Research Center’s Request for Proposal No.
80AFRC18R0006 requires Offerors to identify current or previous customers and solicit their response regarding our performance.
It is requested that the individual(s) responsible for the administrative oversight of the project (e.g. COR, project manager, etc.) respond to this questionnaire.
Upon completion of the survey, please email this survey to: andrea.c.basham@nasa.gov on or before June 10, 2020.
The information contained in the completed Past Performance Questionnaire is considered sensitive and cannot be released to us, the Offeror. If you have any questions about the acquisition or the attached questionnaire, your questions must be directed back to Andrea Basham, the NASA point of contact identified above.
