About this file

This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters for ITB# 269-2025-152, which appears to be related to 2025 Driver Feedback Sign Equipment. The certification form requires bidders, contractors, or subcontractors to disclose and certify their legal and ethical standing in public contracting.

The certification requires disclosure of any debarment, suspension, criminal convictions, civil judgments, or public transaction terminations within the past three years. Specifically, the document asks bidders to confirm they are not currently debarred or suspended from federal or state transactions, have not been convicted of fraud or criminal offenses in public contracting, are not currently under criminal or civil indictment, and have not had public transactions terminated for cause. The form provides two options: certifying compliance or indicating an inability to certify, with the latter requiring an attached explanation. False statements on this certification may result in proposal rejection, contract termination, or potential criminal prosecution.
