About this file

This is an Exhibit B Organizational Chart submission requirement document issued by the City of Detroit Office of Contracting and Procurement (OCP) on behalf of the Construction and Demolition Department for an RFP soliciting Vactor drainage cleaning services. Respondents must submit a portable document format (.pdf) organizational chart for the specific office or unit that will provide services under the contract. If a vendor operates multiple locations across regions or states, only the organizational chart for the office providing services need be submitted. The contract is for a three-year term to provide Vactor drainage cleaning services at various facilities throughout the City of Detroit, located in Michigan.

Respondents must clearly identify vacant or unstaffed positions within their organizational charts. When multiple positions share the same job title, vendors may consolidate the presentation by listing the title with the total number of positions and indicating the quantity of vacancies, rather than naming each individual staff member. For example, if a company has twelve Service Technicians with one vacancy, this may be presented as "Service Technician – X 12, 1 vacancy." This streamlined approach allows vendors flexibility in presenting their staffing structure while maintaining transparency regarding staffing levels and open positions relevant to contract performance.

EXHIBIT B

REQUEST FOR PROPOSALS – VACTOR SERVICES

ORGANIZATIONAL CHART

Please thoroughly read the following instructions and notices before completing this attachment. Interested vendors must ask any questions related to this attachment prior to the deadline identified in this RFP.

Instructions & Notices

The Respondent must provide an organizational chart for the business in a portable document format (.pdf).

If the business entity includes multiple offices/units across several regions/states/countries, the Respondent only needs to submit (an) organizational chart(s) for the office(s)/unit(s) that may provide services under this contract.

For example, 123 Vactor Services solely operates in the State of Michigan. 123 Vactor Services has offices in Detroit, Lansing, Grand Rapids, and Traverse City. However, 123 Vactor Services only expects staff from the Detroit office to provide services under this contract. So, 123 Vactor Services will only need to provide an organizational chart for the Detroit Office.

The Respondent must clearly indicate those positions (if any) in the organizational chart that are vacant/unstaffed.

If a Respondent has multiple positions of the same job title in the organizational tree, the Respondent is NOT required to list all the staff persons under that title. The Respondent may simply list the job/position title and the number of positions under that title. However, the Respondent must indicate the number of vacant positions under that title.

For example, 123 Vactor Services has four Supervisory Technicians who each manage three Service Technicians and their Apprentices. One Service Technician position is vacant and two Apprentice positions are vacant. Therefore, 123 Vactor Services may present that portion of their organizational chart as follows:

Supervisory Technician – X 4

Service Technician – X 12, 1 vacancy

Apprentice – X 12, 2 vacancies
