About this file

This is a Drug-Free Workplace Certification Form required by the State of Florida under Florida Statute 287.087, applicable to the Hillsborough County Water Resources Department's Van Dyke Flow Diversion project. The form requires vendors submitting proposals for the pump station and pipeline infrastructure project to certify compliance with drug-free workplace requirements. Vendors must publish statements to employees prohibiting unlawful manufacture, distribution, dispensing, possession, or use of controlled substances in the workplace and must specify disciplinary actions for violations. Vendors are required to inform employees about drug abuse dangers, the company's drug-free workplace policy, available counseling and rehabilitation resources, and penalties for violations. All employees engaged in providing commodities or contractual services under the bid must receive a copy of the workplace prohibition statement.

The certification requires vendors to notify employees that compliance with the drug-free workplace terms is a condition of working on the project and that employees must report any conviction or guilty plea related to controlled substance violations within five days of occurrence. Vendors must impose sanctions or require satisfactory participation in drug abuse assistance or rehabilitation programs for convicted employees. The authorizing representative must sign and date the form, certifying that the firm maintains full compliance with all stated requirements and makes a good faith effort to continue maintaining a drug-free workplace. This form must be completed and submitted as part of the proposal package for the five-year, $2,000,000 estimated Construction Manager at Risk delivery project.
