The contractor must track and report on multiple performance metrics including the number of clients served, wraparound services offered, consumer satisfaction scores, and ensure that a minimum of 70 percent of unique clients report symptom reduction as measured by the validated assessment tool. Monthly invoices must be submitted through an electronic billing system with detailed service descriptions, dates, contract number, and expense line items aligned with the attached budget. The contractor must be enrolled in the State of Utah's Electronic Funds Transfer (EFT) system for payments, and all billings for services performed in a given fiscal year must be submitted by July 14th of the following fiscal year. Funding allocation, budget, and rate table amendments may be issued by DHHS for governmental entities without requiring contractor signature, and any unexpended funds by the end of the funding period will lapse with no further claims available to the contractor.
