About this file
This is a Request for Proposal (RFP) issued by the Utah Department of Health and Human Services (DHHS) for the Housing and Stabilization Program (DHHS26087), which seeks to provide safe housing and wraparound services for individuals and families leaving polygamous communities. The program aims to increase housing unit availability and expand access to services and supports. Offerors must propose expanding housing capacity to include six new apartments and 17 additional beds by June 30, 2027, while providing comprehensive wraparound services including case management, mental health counseling, peer support services, and educational support services. The contract term extends through June 30, 2027, with evaluation based on five technical criteria addressing community need, housing expansion plans, service delivery, sustainability, and budget justification.
To qualify, applicants must meet two mandatory minimum requirements: demonstrating that their agency has a primary client focus of working with individuals and families leaving polygamous communities, and providing documentation of a minimum of three years' experience delivering direct services to this target population. The total budget is capped at $250,000 through June 30, 2026, and not to exceed $1,250,000,000 between July 1, 2026 and June 30, 2027. Applicants must submit required documentation including IRS Form W-9, conflict of interest disclosures, and a comprehensive proposal narrative with cover page, technical criteria responses, and detailed budget justification. DHHS will only review documentation specifically requested in the solicitation and will not consider extraneous materials submitted by vendors.
