About this file
This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division. The form is designed for potential contractors and bidders to certify their legal and ethical standing when seeking to do business with Los Alamos County. The certification requires offerors to attest that they are not currently debarred, suspended, or excluded from public transactions, and have not been convicted of or involved in fraudulent, unethical, or criminal activities within the past three years.
The document outlines specific disqualifying conditions, including convictions related to fraud, antitrust violations, embezzlement, theft, and other offenses that would indicate a lack of business integrity. Additionally, the form requires bidders to confirm they are not current or former county employees, not immediate family members of county employees or public officials, and have not had public contracts terminated for cause or default within the past three years. Falsification of any statement on this form can result in the non-consideration of a proposal or the rescission of a contract award.
