About this file

This is a Cost Proposal Form for RFP #26LC04P5 (Athletic Partnership for Copper Hills High School) issued by the Jordan School District in Utah. The RFP seeks to establish a partnership for providing athletic uniforms, apparel, facility branding, and merchandising services for Copper Hills High School. The proposal encompasses four distinct components: uniforms and team gear for 29 sports and programs; general apparel and merchandise through an online store; facility branding and visual assets; and promotional incentives or sponsorships. The Core Uniform Package for each athlete includes home and away uniform sets, a warm-up set, and a core practice t-shirt across three pricing tiers (Good, Better, Best). Facility branding items include athlete banners, avenue banners, vertical hanging flags, gameday banners, media backdrops, locker room decals, floor graphics, digital scoreboards, and inflatable tunnels. Offerors may bid on one or more components, and the District reserves the right to award separate contracts to different vendors for different components.

The cost proposal is worth a maximum of 75 points distributed across the components: Uniforms and Team Gear (40 points), Facility Branding and Visual Assets (30 points), and Promotional Incentives (5 points). Scoring for uniforms is based on the highest average percent discount off Manufacturer's Suggested Retail Price (MSRP), with the highest bidder receiving full points and others receiving proportional points. Facility branding scoring is based on the lowest proposed cost, with points calculated as a ratio to the lowest cost. Promotional incentive scoring rewards the highest proposed total value over a five-year period. General apparel and merchandise pricing is required if applicable but not scored. Offerors must complete detailed pricing tables for each sport and tier, include any applicable revenue share percentages for merchandising models, and specify costs for all facility branding items by unit. Cost proposals must be submitted separately from technical proposals, with any inclusion of pricing data in the technical proposal resulting in disqualification for violation of Utah Code Annotated 63G-6a-707(7).

COST PROPOSAL FORM

(75 Points Possible) Offeror Name:

This document to be submitted with your RFP response. The Cost Proposal must be submitted separately from the Technical Proposal. Inclusion of any cost or pricing data within the Technical Proposal will result in the proposal being judged as nonresponsive for violation of UCA 63G-6a-707(7). The JSD Buyer will conduct the Cost Proposal Evaluation independently from the Technical Proposal Evaluation.

This Cost Proposal format is based on the following components. Offerors may submit pricing for one or more of the following distinct components. JSD reserves the right to award separate contracts for separate components to different Offerors to achieve the overall best value. Offerors are not required to bid on all components.

· Component 2: General Apparel & Merchandise/Online Store (required if applicable but not scored)

· Component 3: Facility Branding & Visual Assets (30 points possible)

· Component 4: Total Promotional Incentive/Sponsorship (5 points possible) Fill in the yellow fields for all components the Offeror is proposing. Additional information and the formulas to determine points awarded are listed in each component section.
