About this file
This is a URS Post-Retired Verification Form for Independent Contractors (1099 Employees) related to the State of Utah and Jordan District. The form requires contractors and independent contractors working with Jordan District to disclose whether they have retired from the Utah Retirement Systems (URS). Any individual who has received retirement benefits from URS and is returning to work as an employee or independent contractor for a participating URS employer must complete and submit this form along with a W-9 to the Accounting Department. The form must be signed and dated by the contractor before work begins.
URS retirees returning to work have specific restrictions on reemployment as defined under Title 49 of the Utah Code. Individuals who fail to report reemployment to URS on the Post-Retirement Employment form (Form RTRT-27A) in violation of Title 49, Section 11-1207(2) will be liable to repay URS any overpayment amounts resulting from the failure to report. All contractors must contact URS's Retirement Benefits Department at 801-366-7770 prior to beginning work with Jordan District to complete and file the required Post-Retirement Employment form.
