About this file
This document is a Felony Conviction Notification form from the Amarillo Independent School District, addressing requirements under Texas Legislative Senate Bill No. 1, Section 44.034. The form mandates that any person or business entity entering into a contract with a school district must provide advance notice if the person or an owner/operator has been convicted of a felony, including a general description of the conviction.
The form provides three checkbox options for compliance: (1) the firm is a publicly-held corporation exempt from reporting, (2) the firm is not owned or operated by anyone with a felony conviction, or (3) the firm is owned or operated by individuals with felony convictions, which requires detailing the name and specifics of the conviction. The school district retains the right to terminate a contract if the required notification is not provided or if the conviction details are misrepresented, though the district must compensate the entity for services performed before contract termination.
