About this file
This is a Pricing Schedule attachment for a Request for Quotations (RFQ) for nonpersonal Hospitalist Physician Services. Quoters must submit fully burdened hourly rates for seven ordering activities: Phoenix Indian Medical Center (PIMC) with 6,240 estimated annual hours, Whiteriver Service Unit (WRSU) with 4,152 hours, and five additional facilities (Chinle, Hopi, Elko, Fort Duchesne, and Yuma) each with 2,080 estimated annual hours. The fully burdened hourly rate must encompass all direct and indirect costs including wages, fringe benefits, recruitment, credentialing, insurance, travel, lodging, meals, per diem, relocation, taxes, overhead, general and administrative expenses, and profit. The Government will calculate evaluated amounts by multiplying estimated annual hours by the quoted hourly rate for each ordering activity to determine the quoter's total evaluated price, which serves as an evaluation construct only and does not represent a BPA ceiling, guaranteed expenditure, or obligation of funds.
The solicitation is set aside for Indian Small Business Economic Enterprises (ISBEEs) under the Buy Indian Act and intends to establish one or more Blanket Purchase Agreements (BPAs) through comparative evaluation. Quoters must provide rates in U.S. dollars rounded to two decimal places for all seven ordering activities, and incomplete pricing submissions may be rejected. The Government will evaluate pricing for completeness, mathematical accuracy, reasonableness, and consistency with solicitation requirements. No separate premiums will be paid for night shifts, weekends, holidays, or overtime; all such services will be invoiced at the same fully burdened hourly rate. The estimated annual hours are provided solely for evaluation and acquisition planning and do not represent guaranteed hours, minimum purchases, staffing positions, full-time equivalents, or BPA ceilings.

ATTACHMENT B – PRICING SCHEDULE INSTRUCTIONS
Hospitalist Physician Services
Phoenix Indian Medical Center and Participating Phoenix Area Ordering Activities
Quoters shall complete all required pricing fields in Attachment B. Pricing submitted in Attachment B will be used by the Government for price evaluation and, for successful Quoters, establishment of the applicable fully burdened hourly rates under the resulting Blanket Purchase Agreement (BPA).
1. Fully Burdened Hourly Rates
Quoters shall provide one fully burdened hourly rate for Hospitalist Physician Services for each Ordering Activity identified in the Pricing Schedule.
The quoted hourly rate shall apply to authorized Hospitalist services performed during daytime, nighttime, weekend, and holiday coverage. Quoters shall not provide separate daytime, nighttime, weekend, holiday, overtime, or extended-shift rates.
The fully burdened hourly rate shall include all costs necessary to perform the services, including, but not limited to:
Wages and fringe benefits;
Recruitment and retention costs;
Credentialing and onboarding support;
Professional liability and other insurance;
Travel and transportation;
Airfare and mileage;
Lodging and meals;
Per diem;
Rental vehicles;
Relocation costs;
Taxes;
Subcontractor costs;
Overhead;
General and administrative expenses;
Profit; and
All other direct, indirect, or incidental costs associated with performance.
Government housing is not provided or guaranteed. Quoters are responsible for considering the geographic location, remoteness, availability and cost of lodging, transportation requirements, and other location-specific conditions when developing their rates.
Unless expressly provided otherwise in the BPA, travel, lodging, transportation, meals, per diem, and related expenses will not be separately reimbursed.
2. Ordering Activity Rates
The Pricing Schedule identifies the Phoenix Area Ordering Activities for which rates are requested.
Unless the Pricing Schedule expressly establishes a separate facility-specific rate, the quoted hourly rate for an Ordering Activity shall apply to Hospitalist services performed at the primary and affiliated facilities identified under that Ordering Activity in the PWS.
Quoters should review Section 3 of the PWS, Ordering Activities and Included Facilities, before developing their rates.
3. Estimated Annual Hours for Price Evaluation
The estimated annual hours shown in the Pricing Schedule are provided solely for price evaluation and acquisition planning.
The estimated hours:
Do not represent guaranteed hours;
Do not represent minimum purchases;
Do not represent staffing positions or full-time equivalents (FTEs);
Do not establish a BPA ceiling;
Do not obligate funds; and
Do not guarantee issuance of a BPA Call.
Actual Hospitalist coverage requirements may be greater or less than the estimated hours used for evaluation. Actual authorized hours, schedules, periods of performance, places of performance, and funding will be established through individual BPA Calls.
4. Evaluated Amount
For each Ordering Activity, the Government will calculate the evaluated amount as follows:
Estimated Annual Hours × Quoted Fully Burdened Hourly Rate = Evaluated Amount
The Government will sum the evaluated amounts for all Ordering Activities to determine the Quoter's Total Evaluated Price.
Total Evaluated Price is an evaluation construct only. It does not represent a BPA ceiling, guaranteed expenditure, minimum purchase, or obligation of funds.
5. Extended Shifts and Additional Authorized Hours
No separate premium will be paid for nights, weekends, holidays, overtime, or extended shifts.
When authorized Hospitalist services extend beyond a scheduled shift, the additional authorized hours will be invoiced at the same fully burdened hourly rate applicable to the Ordering Activity.
The Contractor shall not perform or invoice hours exceeding the authorized coverage or funded ceiling of a BPA Call except as expressly authorized by the Contracting Officer or otherwise permitted by the terms of the BPA Call.
6. Pricing Completeness
Quoters shall provide a fully burdened hourly rate for each Ordering Activity identified in the Pricing Schedule. Failure to provide required pricing may result in the quotation being considered incomplete.
Rates shall be entered in U.S. dollars and rounded to two decimal places.
The Government will evaluate pricing for completeness, mathematical accuracy, reasonableness, and consistency with the solicitation requirements.
