B. EXECUTIVE SUMMARY

This Environmental Inspection Report has been conducted in accordance with the Scope of Work for the project as defined by the Indian Health Service Division of Facilities Management.

Kingston Environmental Services, Inc. (KES) of Lee's Summit, Missouri served as a subcontractor to Martell and Associates, P.A. of Kansas City, Kansas.

The purpose of this Environmental Inspection was to determine the present locations and conditions associated with the following environmental parameters, as defined in the Scope of Work:

• Asbestos-Containing Materials (ACM)

• Lead-Based Paint (LBP)

• Polychlorinated Biphenyls (PCB's)

This Executive Summary has been organized in accordance with Section III through VII of the Scope of Work.

Section III - Asbestos-Containing Materials

Bulk Material Sampling and Analysis

Asbestos-containing materials (ACM) are represented in three major categories;

• Surfacing materials; sprayed-on or troweled-on surfaces such as walls, ceilings or structural members.

• Thermal system insulation; on pipes, ducts and heating/air conditioning equipment.

• Miscellaneous materials; such as floor tile, ceiling tile, sheetrock and cement asbestos panels.

A total of 54 bulk asbestos samples were retrieved from the seven (7) different structures that were inspected for asbestos. The sampling protocol utilized meets the requirements of the Asbestos Haz.ard Emergency Response Act (AHERA). The analysis performed on the samples was performed by the Kingston Environmental Laboratory (KEL). KEL is currently a National Voluntary Laboratory Accreditation Program (NVLAP) accredited laboratory. The analytical method, USEPA Method 600/R-93/116, was used to determine the presence or absence of asbestos in the samples retrieved. Each structure has been given a designated number which corresponds to the site plan, included herein. The ACBM Schedule defines the floor number, space description, homogeneous material type, quantity, size, percent damaged, sample number and general notes associated with the suspect asbestos-oontaining material.

I '

Ambient Air Samples

A total of eight (8) ambient air samples were retrieved from various areas where friable asbestos contajning materials were observed. These locations included the maintenance room and select hallways on the first and second floors. Each sample was taken in a location w~ere suspect thermal system insulation was present. All samples collected between 1,280 and 1,450 liters of ambient air for time periods ranging from 128 to 145 minutes. The analysis of these samples was performed at the KEL by an analyst who is currently participating in the American Industrial Hygiene Associations (AIHA), Asbestos Analysts Registery (AAR). Fiber counts ranged from

0.003 fibers/cc to 0.014 fibers/cc. The results of these ambient air samples indicate that the concentrations of airborne fibers present within the areas sampled do not exceed the present federal action level of0.l fibers/cc as determined through NIOSH Fiber Counting Method 7400.

Locations of Positive ACM

The following is a summary listing of the locations and materials that were found or assumed to contain asbestos:

• Building# 421 (Main Hospital) - Thermal system pipe insulation, fire doors, linoleum, floor tile, cement asbestos board, duct insulation, duct connectors and debris.

• Building# 422 (Material Management) - No asbestos containing materials.

• Building# 423 (Office of Environmental Management and Maintenance) - Floor tile mastic.

• Building# 415 (Snack Bar) - Linoleum and surfacing material.

• Building# 414 (Personnel) - Surfacing material and linoleum.

• Building # 413 (Mental Health) - Surfacing material, linoleum and roof flashing

• Building# 412 (Audiology) - Surfacing material, window glazing, roof flashing and exterior window caulking.

Diagrams of ACM Sample Locations

Diagrams of the locations where ACM samples have been retrieved are located in the asbestos containing materials section of this report (Section C).

Health Risk Evaluation

At present, several areas will require immediate removal of asbestos-containing thermal system insulation. These areas where asbestos~containing materials are significantly damaged are likely to continue to degrade and present a health hazard to the occupants of these areas of the building.

B-2

I , These areas are as follows:

• Building# 421 - Maintenance Office damaged thermal system insulation.

• Building# 421 - damaged thermal system insulation above drop ceilings.

Abatement Feasibility Options

Abatement (Removal and Repair) of asbestos-containing materials found within the various structures would serve as a sufficient method for the rectification of the potential health haz.ard associated with damaged asbestos-containing materials and occupant exposure. Complete removal of all asbestos-containing materials would be the optimal method for remediating the health hazard. Isolation of the work areas where asbestos abatement would occur will be necessary during the abatement work.

Abatement Recommendations

KES suggests that all ACM be removed from the Lawton Indian Hospital Buildings, however, if budgetary constraints prohibit this, we recommend that select removal of unrepairable asbestos containing materials be performed in conjunction with repair of the remaining damaged ACM.

Cost estimates associated with complete abatement, as well as select abatement work, have been included for the applicable buildings, in the conclusions and recommendations section of this report (Section F).

Sampling and Analysis Procedures

All sampling and analysis procedures confonn to the AHERA protocol as defined in 40 CFR Part 763 and 29 CFR 1910.1001.

Bulk Sampling Results

A total of fifty-four (54) samples were retrieved from the seven (7) buildings on the hospital campus. Twenty-three (23) of these samples were analyzed as containing asbestos at a level greater than 1 %.

Air Sampling Results

A total of eight (8) ambient air samples were retrieved from areas where friable ACM was present. The results of these samples ranged from 0.003 fibers/cc to 0.014 fibers/cc. The results of these ambient air samples indicate that the concentrations of airborne fibers present within the areas sampled do not exceed the present federal action level of 0.1 fibers/cc as determined through NIOSH Fiber Counting Method 7400.

B~3

Section IV - Lead-Based Paint

LBP Design and Testing

The sampling protocol utilized was an abbreviated form of the HUD protocol for the inspection oflead-based painted items, 24 CFR Part 35. This abbreviated form did not incorporate the testing of every room within each structure. Testing was performed on select components and assumptions as to similar painting histories were made. 1hls testing was performed on all of the different components found within each structure.

Field Testing Instrument

KES utilized the Scitec Map 4 X-Ray Fluorescence LBP analyzer in conjunction with the Performance Characterization Sheet (PCS) for this model XRF. This spectrum analyzer was used for all areas where suspect LBP exists.

Sampling Design

This information has been included with the pre-work submittals for the project.

XRF Sampling

All painted interior and exterior component types have been tested in accordance with the Scope of Work for the project. The number of locations where each component type have been tested will vary from the requirements of 24 CFR, Part 3 5.

Laboratory Testing

A total of two (2) confirmatory laboratory samples were retrieved from locations where inconclusive XRF results were observed. The following are the results of the two samples retrieved:

Building# 415 - Dining Room N. Baseboard Building# 414 - S. Office S. Wall

0.059 ppm (Negative for Lead)

0.751 ppm (Positive for Lead)

Each of these samples were labeled, properly packaged and submitted to Schneider Laboratories ,. • of Richmond, Virginia. Schneider Laboratories is presently accredited under the AIHA-ELLAP program as required by the Scope of Work. A copy of the Schneider Laboratories, Inc., current accreditation has been included in Section G of this report.

Test Records

A complete set of the XRF results and laboratory confirmation sampling has been included in section D of this report.

B-4

Section V - Polychlorinated Biphenyls

In accordance with the RFP, Investigative Study of Polychlorinated Biphenyls (PCB's), the following summary of results for tasks A and B have been provided:

KES performed a visual evaluation of the fluorescent light ballasts and transformers to detennine whether this equipment was suspect to contain PCB's. A total of thirteen (13) different light fixture types were observed within the seven buildings. The number of each fixture type present in each building was recorded. A representative number of each fixture type was disassembled and the ballast inspected for NON-PCB labeling. Ballasts which did not contain the NON-PCB labeling were assumed to contain PCB's in accordance with 40 CPR Part 761. Additionally, it was assumed that all fixture types had the same type of ballast transformers present.

No testing of the ballasts or the transformer's dielectric fluid was performed by KES.

B-5

SECTIONC

C. ASBESTOS-CONTAINING MATERIALS

In accordance with the Scope of Work for this environmental inspection, suspect asbestos containing surfacing materials, thermal system insulation and miscellaneous materials located throughout the facility were identified and surveyed. These materials included, but are not limited to, ceiling textures and spackling (surfacing materials); pipe insulation, hard fittings on fiberglass and tank insulation (thermal system insulation); floor tile, floor tile mastic, ceiling tiles, covebase and mastic, linolewn, and window glazing (miscellaneous materials).

Materials sampled in a previous inspection dated October, 1999, that were clearly identifiable, were not to be re-sampled. The previous inspection report listed sixty (60) samples from building# 421 (Hospital) of hard wall plaster and drywall as negative for asbestos content. For this reason, hard wall plaster and drywall were not sampled in this inspection.

Materials uniform in color and texture were grouped into homogeneous areas. Bulle sampling was conducted in a manner representative of suspect materials in each functional space. Bulk samples were placed into plastic containers, sealed, labeled and transported to the Kingston Environmental Laboratory for analysis. The location of the functional space and homogeneous areas, the amount of materials in each homogeneous area, and the locations at which the samples were collected have been recorded.

Analysis was performed by polarized light microscopy (PLM) in conjunction with dispersion staining techniques according to the EPA Interim Method for the Determination of Asbestos in Bulk Samples, EPA-600/R-93/116. This method is sensitive to the presence of asbestos at less than one percent by volume. This inspection refers to samples with negative or positive results.

A negative sample is one which no asbestos is detected or less than 1 % asbestos is present. A positive sample is one which contains greater than 1 % asbestos by volume.

The following tables are attached, showing building finish materials sampled, listings of bulk sample codes for each building and listings of finish materials which tested positive, as asbestos containing. Tables are as follows:

Homogeneous Material Code List - A listing of suspect materials found in each building for which bulk samples were taken.

ACBM Schedule - A listing of each of the suspect asbestos-containing materials cited, bulk samples taken by building, showing floor number, space description, homogeneous material code, quantity, size, percent damaged, sample number and detail notes. Note - shaded areas indicate materials which tested positive as asbestos-containing.

Note: Kingston sample numbers are defined below in "Sample Identification". A previous inspection performed internally by IHS personnel utilized a different sample identification scheme such as 412-04-06. The "41211 is the building nwnber, "0411 is the homogeneous material number and "0611 is the sample number. The IHS sample numbers are referenced in the ACBM Schedule to show that each homogeneous material has been sampled. Sample numbers are shown in the area where they were

C-1 actually collected. If the homogeneous material associated with the sample number was analyzed as positive, AIIERA rules state that all like (homogeneous) materials are assumed to be positive also. Hence the name "homogeneous material".

Bulk Sample Analysis Report - An analytical report listing the asbestos sample number, material description, sample location and a breakdown of the sample constituency as well as a determination as to whether the material is an asbestos-containing material.

Sample Identification - Sample number 421-MJFI-01 can be interpreted as follows:

421 Building Number MJFl Homogeneous Material Code 01 Sample Number

Ambient Air Sample Report - A copy of the ambient air sample reports defining the sample number, location, type, flow rate, volume and actual fiber concentration has been included in this section.

The following floor plan drawings and digital photographs are attached, showing the locations of the samples retrieved as well as a photograph of each sample taken.

Floor Plan Drawings - CADD drawings for all floors of each building where suspect asbestos containing materials were inspected have been included with this report (Section C). These floor plans list the locations where asbestos-containing materials have been sampled including the sample number and an arrow designating the location of the sample.

Sample Photographs - A digital photograph of all samples taken from each building have been included. A dry erase board has been placed in the photograph to designate the sample number associated with the sample type.

The following is a summary list of the seven (7) structures located on the Lawton Indian Hospital Campus that were inspected for suspect asbestos-containing materials. Each building's sampling and testing data has been listed separately to provide a more user friendly format.
