About this file
This document contains a monthly report template for contractors conducting tobacco retailer inspections under an FDA contract. The template requires contractors to provide detailed data on inspections completed during the reporting period, including the total number of retail establishments, inspections assigned and completed, training provided, media coverage and coordination with other tobacco control programs. Contractors must also outline their retail inspection coverage plans for the previous and upcoming months. The template indicates contractors will inspect retailers near schools and in minority communities and report on program coordination, property management and feedback on the inspection process. Contractors must submit a completed report template to FDA on a monthly basis to detail performance under the tobacco retailer inspection contract.
