About this file

This document contains tables from an attachment to a Request for Proposal seeking to increase equitable access to COVID-19 testing. The tables require offerors to provide pricing details for various mandatory and optional test types that could be performed at mandatory settings including pharmacies, surge sites, and drive-thru locations, as well as optional settings like federally supported sites and congregate locations. Test types include swab and send, unobserved self-collection, point-of-care NAAT, antigen, and sequencing options. Pricing and maximum test numbers must be provided for the base period and five option periods. Table 6 requires the total proposed costs to be calculated by multiplying the test prices by the maximum quantities. The Centers for Disease Control and Prevention is seeking these proposals through Solicitation Number 75D301-22-R-72105 as part of their Increasing Community Access to Testing program. Questions are due by February 8, 2022.
