About this file
This document contains two tables providing templates for offerors to propose testing costs and testing approaches for a federal contract soliciting COVID-19 testing services. Table 4 requests pricing for various test types including swab and send laboratory tests, home sample collection, point-of-care, confirmatory, pooled, and genomic sequencing tests. Table 5 requires offerors to indicate the number of tests proposed across mandatory and optional testing settings such as pharmacies, surge sites, drive-thrus, and congregate facilities. Table 6 directs offerors to use the template to propose total costs for the base period and five option years across mandatory and optional tasks including sample accessioning, wrap-around services, and federally supported and congregate testing.
