About this file

This document is a Request for Proposals (RFP) issued by the State of Utah Division of Purchasing for wheelchairs and power-operated vehicles. The solicitation (CT19051) was open from February 1, 2019, to March 8, 2019, with proposals due by 2:00 PM Mountain Standard Time. The contract is anticipated to result in multiple awards per region, with a five-year term from the award date. The State seeks to establish a contract for eligible users to purchase wheelchairs and power-operated vehicles on an as-needed basis, with the current portfolio set to expire on June 30, 2019.

The RFP includes mandatory minimum requirements for vendors, including having at least one storefront location in Utah, maintaining an inventory for testing, having a certified mobility specialist, and meeting the specifications outlined in the Scope of Work. Vendors must provide detailed information about their business, including contact details, tax identification numbers, and regions of service. The evaluation will be based on meeting minimum required scores, with cost proposals evaluated independently. Vendors are required to submit various attachments, including a completed Cost Sheet, and may offer special pricing for educational entities. The State does not guarantee any purchase amount under the awarded contract, and multiple contract awards are anticipated based on meeting the minimum required score.
