PAST PERFORMANCE QUESTIONNAIRE
Solicitation: 70Z04422RESD30016 C4IT Engineering Development, and Integration, Support Services (CEDISS)
The purpose of this questionnaire is to obtain information regarding the quality of the Contractor’s past performance relative to a contract, completed or in progress, at your company/agency. The Department of Homeland Security (DHS), U.S. Coast Guard (USCG), Command, Control, Communications, Computer, Cyber and Intelligence (C5I) Service Center (C5ISC) is considering this Contractor for C5ISC Engineering Development, and Integration, Support Services (CEDISS). We would greatly appreciate your input regarding the quality of the Contractor’s past performance.
Handwritten responses are sufficient. It is requested that the individual(s) responsible for the administrative oversight of the project (e.g. security specialist, COR, COTR, etc.) respond to this questionnaire. More than one copy of the questionnaire may be submitted if responses from more than one individual are appropriate.
Upon completion of the survey please email to TIS-SMB-C5I-9143-3-Branch@uscg.mil and provide a copy to the Contract Specialist at bobbi.j.forbes@uscg.gov, and the Contracting Officer at cheryl.r.ellis@uscg.gov . Section I - Contract Information to be completed by the offeror The Department of Homeland Security (DHS), U.S. Coast Guard (USCG), Command, Control, Communications, Computer, Cyber and Intelligence (C5I) Service Center (C5ISC) has a requirement to procure C5ISC Engineering Development, and Integration, Support Services (CEDISS) for the design, development, integration, testing, and operational evaluation of new C5I and electronic systems as well as modifications and technology refreshment or upgrades to existing C5I and related electronic systems. These services include the development and maintenance of engineering change and logistics support documents that enable C5I and electronic system sustainment.
