Please assess each of the seven (7) following areas with a rating that is based on the provided objective measurable performance indicators to the maximum extent possible, Commentary to support the rating for the element assessed where indicated at the end of each section. Assign each a rating among the choices of Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory. Use the below rating guidelines as guidance in making these evaluations. Insert the appropriate rating.
Exceptional – Performance meets contractual requirements and exceeds many to the Government’s benefits. The contractual performance of the area being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective. There are multiple significant events identified relating to the area being assessed with detail as to how they were of benefit to the Government. A singular benefit, however, could be of such magnitude that it alone constitutes an
Exceptional rating. Also, there have been no significant weaknesses identified.
Very Good – Performance under the area being assessed meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the area being assessed was accomplished with some minor problem for which corrective actions taken by the contractor was effective.
There is a significant event identified relating to the area being assessed with detail as to how it was a benefit to the Government. There have been no significant weaknesses identified.
Satisfactory – Performance under the element being assessed meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory. For the element being assessed, there have been only minor problems, or major problems the contractor recovered from without impact to the contract/order. There have been NO significant weaknesses related to the element being assessed. The contractor has not been assessed a rating lower than Satisfactory solely for not performing beyond the requirement of the contract/order.
Marginal – Performance under the element being assessed does not meet some contractual requirements.
The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented. For the element being assessed, a significant event has been identified in each category that the contractor had trouble overcoming and details provided as to how it impacted the Government. A reference to the management tool that notified the contractor of the contractual deficiency (e.g., management, quality, safety, or environmental deficiency report or letter) should also be provided.
Unsatisfactory – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective. For the element being assess as multiple significant events have been identified in each category that the contractor had trouble overcoming and details provided as to how it impacted the Government. A singular problem, however could be of such serous magnitude that it alone constitutes an unsatisfactory rating. A reference to the management tools used to notify the contractor of the contractual deficiencies (e, g. management, quality, safety, or environmental deficiency reports, or letters) should also be provided.
