About this file

This is a Reference Questionnaire document for the State of Nevada Purchasing Division, submitted on behalf of the Nevada Department of Conservation and Natural Resources (DCNR), Nevada Division of Environmental Protection (NDEP), and the Bureau of Mining Regulation and Reclamation (BMRR). The solicitation seeks proposals from qualified vendors to provide Certified Environmental Manager services for mine site interim fluid management operations. The document appears to be a standardized questionnaire designed to gather information from prospective bidders regarding their qualifications, experience, and references relevant to environmental management services in the mining sector.

The questionnaire likely requests detailed information about the vendor's background, certifications, past project experience, financial stability, and references from previous clients. This reference document serves as an evaluation tool to assess bidder qualifications and capability to perform the specialized environmental management services required by the State of Nevada. The specific pricing terms, contract duration, renewal options, and submission deadlines would be detailed in the accompanying RFP and Scope of Work documents referenced in the solicitation materials.
