US DEPARTMENT OF HOMELAND SECURITY

IMMIGRATION AND CUSTOMS ENFORCEMENT

PAST PERFORMANCE QUESTIONNAIRE

PART ONE: INSTRUCTIONS

[Offeror] is proposing on a U.S. Department of Homeland Security (DHS), Immigration and Customs Enforcement (ICE) Indefinite Delivery Indefinite Quantity (IDIQ) request to provide Detention/Transportation/Food Services to meet the mission needs of ICE. [Offeror] has provided ICE with your contact information as a reference for their Past Performance. Past Performance is an extremely important part of the evaluation criteria for this acquisition, so your input is very important.

The information contained in this Past Performance Questionnaire is to be provided directly to the OAQDCRSW@ice.dhs.gov. This information will not be disclosed to the respondent. Please provide an honest assessment of the vendors performance and return this questionnaire to ICE no later than [TBD] email to OAQDCRSW@ice.dhs.gov. If you have any questions, please contact Natalie Carr by email at Natalie.Carr@ice.dhs.gov.

Please utilize the following rating system during the completion of this questionnaire:

Code Rating Description E Exceptional Performance meets contractual requirements and exceeds many

(requirements). The contractual performance of the element being assessed was accomplished with few minor problems for which corrective actions taken by the contract were highly effective.

VG Very Good Performance meets contractual requirements and exceeds some (requirements). The contractual performance of the element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

S Satisfactory Performance meets contractual requirements. The contractual performance of the element being assessed contains some minor problems for which correcting actions taken by the contractor appear or were satisfactory.

M Marginal Performance does not meet some contractual requirements. The contractual performance of the element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions, or the contractors proposed actions appear only marginally effective or were not fully implemented.

U Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element being assessed contains serious problem(s) for which the contractors corrective actions appear or were ineffective.
