About this file

This is a Small Business Utilization Form issued by the City of Fort Worth, Texas, for the PW 2026 Concrete Restoration Contract 3 (City Project No. 106668). The form establishes the City's small business contracting goal of 30 percent and requires all prime bidders and offerors to identify their commitment to utilizing certified small business firms. The project involves concrete pavement work, including approximately 11,260 square yards of 9-inch concrete pavement, 9,472 square yards of 6-inch concrete pavement, 4,100 cubic yards of flex base, and 18,250 square yards of geogrid installation. All small business firms must be certified by an acceptable agency—including the North Central Texas Regional Certification Agency (NCTRCA), Women's Business Council - Southwest (WBCS), Dallas/Fort Worth Minority Supplier Development Council (MSDC), or Texas Department of Transportation (TXDOT)—and located within the marketplace comprised of Denton, Parker, Wise, Tarrant, and Johnson counties at the time of bid submission.

The form mandates that prime bidders identify all subcontractors and suppliers regardless of certification status and complete the utilization schedule in its entirety as a condition of bid responsiveness. Prime contractors must list the dollar amounts for certified prime bidder services, small business subcontractors and suppliers, and non-small business subcontractors and suppliers, with the total contract value calculated from all tiers. Bidders are prohibited from making additions, deletions, or substitutions to the certified list without prior City approval, with violations constituting material breach of contract and potential grounds for debarment. The form authorizes the City to conduct audits, examine books and records, and conduct interviews with owners, principals, and employees to verify actual work performed by small business firms, with intentional misrepresentation of material facts subject to debarment for a minimum of one year.
