About this file

This is a Hours and Personnel Form for janitorial services submitted by the State of Utah's Division of Facilities Construction and Management (DFCM) as part of a Stage II Invitation to Bid for janitorial services at Logan Workforce Services. The three-year contract requires qualified bidders to have completed Stage I prequalification and be currently listed on the DFCM Prequalified Janitorial Contractors list. The form serves as a detailed staffing and scheduling document that bidders must complete to outline proposed personnel assignments, work hours, shift allocations, and labor deployment across the contract term. This document is essential for demonstrating bidders' capacity to meet the facility's janitorial service requirements through specified staffing levels and operational scheduling.

The Hours and Personnel Form functions as a critical evaluation tool for assessing bidder qualifications and operational feasibility during the competitive bid process. Completion of this form with accurate, realistic, and compliant staffing plans is mandatory for bid submission. The form directly impacts pricing proposals, as labor costs represent the primary expense component in janitorial service contracts. Bidders must ensure their proposed hours and personnel allocations align with the scope of work requirements and facility needs at Logan Workforce Services while maintaining cost competitiveness within the estimated annual budget parameters established by DFCM for this three-year contract award.
